Saved, Loaded and then Immediately Defeated

Sometimes, when I load a saved date, my PC is defeated immediately. As usual hit-point death does not end the game, I guess that is something due to spirit energy.
I am using Makeover version of the MotB in the NWN Vault. And also using several overrides such as Kaedlyin's PRC pack, Tony K's AI, new Safiya by lukasm568, Silver Sword Fix by SushiSquid, and Two-Rowed Hotbar by JOG.
So, maybe some of those overrides are causing this trouble.
But anyway, if some of you have the same experience and/or some knowledge about avoiding this, would you please share your knowledge?

Another instance where this happens seems to be if you saved with certain lingering spell effects (one I remember is the shadow simulacrum cast on you in the wizards' cabin, which produces other odd effects as well). In general, any save during a battle or in the middle of a script ("Leldon's got a bounty on you, goat girl") or some effects seems doomed one way or the other and I avoid them completely.
Also, the game doesn't seem to like you to have too many characters. The standalone mod that allows all the npcs, the db companions mod and/or Kaldor's makeover might not account for some check that comes up occasionally.

Any constitution item equipped?
Yes. And I know that Kaedrin's PRC pack causes positive energy damage if a character is wearing some constitution items. But in this case the amount of the damage should not be that much. My PC is a fighter type and 3-4 hp per level should not kill him.

I've had that happen when modding the saved game files outside the game. A solution that has worked fairly consistently (but not perfectly) is to touch all the files so they have the same modified date.

No, I did not modified the files. Just saved the game and then loaded.
Too many characters could be a possible cause. When playing MotB I let PCs and companions had a lot of summons (elementals and Telthor Companion from Kaedrin's) to gain more spirit energy when suppress.
