Does damage go off of your base score or your modified score? I just got level 7 and I have 2 more DEX modified, so i'm wondering if it's modified or base which is used to determine damage.

You mean dex from gear and stuff? They count. The only time base is used is for skill/talent/spell pre-requisites.
Having +stats on equipment wouldn't be very useful if they weren't used.
