Just as the title says, what has been the most heart-pounding, edge-of-your-seat moment in gaming?
The one that comes to mind for me, alothugh I experienced it rather recently, would be the hotel basement in The Last of Us.

One moment is when I played a match of search and destroy in Modern Warfare, and I was eventually the last one on my team against four on the other team. Each team had 2 points, so this was the final round. I took down three of them, sneaked over to their bomb, defused it, and my team and I won the match.

Probably when I decided to run into the Deathclaw nest at the Quarry south of New Vegas, at level 4 with a 10mm pistol and some Leather Armor.
I didn't kill them all, but I got out of there.
Or some scene in the '13 Tomb Raider.

This happened to me while I was playing a 4v4 game of StarCraft II.
It had been a long and drawn out fight, both sides had their ups and downs, but their forces were but shadows of their former selves. Many powerful armies lived and died, and the map's resources had been all but depleted in the seemingly endless stalemate, but it still seemed that defeat was inevitable. By the time the last of our defenses fell, a few remnants of my team's army were in the enemy base, leveling the place as quickly as they could, but the enemy team's much larger force was already in our base destroying our structure far more quickly. They had already destroyed my Red Zerg teammate's base when my last Ghost arrived on the scene. I remember how I led my shot by predicting how quickly the enemy force would destroy some of my Blue Zerg teammate's structures and predicting where they would position themselves to attack the next building before hiding the laser dot under a pile of zerg guts and dropping the bomb, and sure enough, BOOM! Killed almost every single one of them. Two Thors survived, but there HP was low enough for my Ghost to finish them off with his rifle. And we won! I was laughing so hard when I watched them burn, it was simply glorious.

Many PvP and boss battles from Dark Souls.
Nearly every boss on Bitterblack Isle in Dragon's Dogma.
Battle of Denerim, Dragon Age: Origins. Mainly due to the build up.
Kolgrim fight in Dragon Age: Origins. It's one of my favorite enemy encounters in the game.
Many missions in the Kingdom Under Fire series.
Invading the Nilfgaard Camp in The Witcher 2.
The bosses at the top of the Iron Throne tower in Baldur's Gate.
Mount and Blade battles where it's simply been me and my officers fighting against numerous enemies and winning.
High level character Mount and Blade battles where my character is able to single handily defeat an army of 100 people by himself.
End boss battle of Dragon's Dogma: Dark Arisen deserves a special mention even though I technically mentioned it above.
Forgotten One boss battle in Castlevania: Lords of Shadow and many scenes in the overall series of LoS thus far.
Fighting Death in Castlevania 1. Damn that guy.
Some impressive take-downs I set up in Deus Ex: Human Revolution.
Boss encounters in Divine Divinity. They were quite hard.
Shalebridge Cradle in Thief Deadly Shadows and The Haunted Cathedral in the first game.
Many scenes from the Halo series and many enemy encounters on Legendary difficulty and awesome multiplayer kills I made. Still got them saved.

Jedi Knight 2 was full of that. Everything is Epic about this game.
It seems Bioshock Infinite must have moments like that too.
CoD: WaW has its moments too.
Heck most of the games have one or two moments at least.

Don't think I could pick one. The Bear Chase("RUN FOR YOUR LIFE!") In Condemned 2: Bloodshot. The entirety of Dyad. The fight with Giygas in Earthbound is pretty intense. The first section of God Of War 3, the climbing of mount olympus. It seemed like the majority of its budget went into that one area
My first Assassination in Assassins' Creed. The Jenny Scene in The Darkness. Fighting Frost in Fight Night Champion, that's a grueling fight. My first run in Mirror's Edge, it's a very intense game. Bellfry Gargoyles, Ornstein & Smough in Dark Souls. Meeting Ragnaros in World of Warcraft.

Not a moment per-se, but the war I had playing as the Eastern Roman Empire against the Ghorids Sultanate in Broken Crescent (Medieval II: Total War) certainly stood out as intense. One of the best gaming experiences I ever had tbh.

One of the vaults in FO:NV on Hardcore mode got pretty intense - I was suffering a concussion, limping from a broken leg, and slowly getting weaker from radiation sickness, and without enough medical supplies to do anything about it. Eventually I gave up whatever I was trying to accomplish down there and simply made a beeline for the exit in the hopes that my RAD count wouldn't hit 1000 - and failed. I ended up having to go back to an earlier save after realizing that it just wasn't possible to move fast enough to get out alive.

I haven't had a game with that effect on me in a long time. I remember the first Resident Evil scared the pee out of me. And there's always the fear of Deathclaws which i've had since the original Fallout.

Cutting off Dethmold's balls in The Witcher 2.
A lot of Legendary Pokemon battles were probably the first things I played that I'd call 'intense.'
I remember me and my buddy playing Battlefront 2, where we were the last units on our team while the enemy had over 70 left. We pulled that sheeyut off, and it was awesome.
I've always liked that moment where you're about to duel Loghain in DA:O, and everyone goes quiet and steps aside while you have the classic pre-fight slow circle walk.

Pretty much anything with a countdown timer that usually leads to a giant explosion or something that results in a game over.
I remember in one of the Onimushas there was a room filling up with water and you had to solve a sliding block puzzle or something. Yep, a quick google gave me this.

The Suffering game. Boy, I was not prepared for those screamers, the hallucinations and that god damn haunted mansion.

Then there is Nemesis in Resident Evil, I remember hm crashing through a wall and that ****ing ghost in the mirror in Alone in The Dark: A new Nightmare. I had to turn it off because of that.
Despite its reputation, Kane and Lynch 2:Dog Days. The scene where they were tortured and Lynch's wife being raped and then killed in a dirty toilet covered with her face in some twisted position, then running through the streets naked and covered in blood is incredibly grim, nauseating, depressing and just dark. I felt dirty after that scene, the atmosphere is not nice at all. Manhunt vibes all over.
