It actually would be awesome if characters like Krogans could pick up enemies and throw them at other enemies. Or fastball special a vorcha into enemy traffic.

I wish there would be 3 types of weapons. Guns for those that want to shoot everything. Omni-tools for those that want to play only with powers (biotic or tech). Melee weapons for those that want to impale or decapitate everything.
There could be some cross-play between those, I would love to see Pull + Melee punch detonates a biotic explosion...

All I want for next effect, that it would be at least a bit lore-wise. Which would prevent the things you've listed from existance

Could you be more specific? What isn't lore-wise? Guns? Melee weapons? Omni-tools?
If you're going for the omni-tools affecting the powers, just see it as a boost/regulator for your implants. There, fixed it for ya...
Also, "lore-wise" is taken out of proportion way too often. I hope you're not the kind of guy that reads every fine prints in a game and find 2 things in 100k lines of text that don't fit together and gets upset... After seeing the number of things that makes no sense in ME3, "lore-wise" is a really subjective matter...

I used to think "Man, the Fury shouldn't have to carry a gun, she's a machine." but now I'm glad I'm forced to carry a gun because I learned how to mix powers and weapons and learned to play the game better for it.

Doesn't matter at point blank range. Would be awesome on Vanguards... Dual wielding punishers.. ohh yeah. How about double pistols with bayonets on a Shadow type character? Dual wielding rocks... L2P scrub.
