I haven't experienced that many instances of blank shots. I can only recall 1 or 2 that are actually consistent. The only troublesome bug is the freezing glitch that occurs after Citadel II. ME2 has the most annoying issue for me so far, and it's the audio bug that cuts out the last word during conversations.
Why is ME3 so buggy?

I didn't have all that many issues with me1, one or two with the second one. me3 had a few more... the Normandy cockpit bug, Ashley's marksman bug, the from ashes dlc made a war asset on a small planet uncollectable (planet brin or brinn, something like that), there seems to be vo choppiness after a few of the DLCs were installed, choppiness that seems to get worse as you install more dlcs (though I seem to be the only person remarking about such things as far as I can tell... and I think me2 had a similar issue)
I think I fell through a mesh in the spectre office in the embassy once as well (ME3). but most of all I seem to have issues with the combat interface while on missions, I'd have to click weapon or tech/biotic power icons repeatedly at times, have to push the dodge button several times (a few times fighting banshees & brutes 7 taps on the dodge button didn't cause my character to dodge), powers being triggered but the action not being carried out (like selecting the singularity power and getting hit with the cooldown timer; however, your character didn't launch a singularity). but keep in mind that me1 had a 3 year production cycle, where as mass effect 3 had about 6 months (iirc).
