My very first try? Gosh, that was so long ago... I don't know if I can remember enough of it. I ended up redoing my first Warden's game, and got a much better outcome (or at least, my character was much more refined, and had a proper personality).

Overall, I don't think my choices weren't much different.

Origin: City Elf (male; Orian Tabris; romanced Leliana)
Circle of Magi: Saved the mages with the Litany of Adralla
Redcliffe: Connor and Isolde both survived; the townsfolk mostly survived
The Brecillian Forest: Sided with the elves and the Grand Oak Tree
Orzammar: Sided with Bhelen and Caridin
The Landsmeet: Won the Landsmeet and beat Loghain 1 on 1 (in the original, I failed to convince enough people and had to fight Loghain with his troops), and had to kill Ser Cauthrian (only ever once managed to prevent fighting her... I forget which of my Wardens it was)
Dark Ritual: Morrigan performed the ritual with the Warden.
The Battle for Denerim: The Warden slew the archdemon.
End Game: Anora and Alistair wed and lead Ferelden together.

Made Alistair King, couldn't kill Loghain(I have no idea why I made Alistair do it, I should just did it) Made Alistair do it and in turn, he couldn't marry Anora.

The difference with my canon is that Loghain lives and Alistair rules with Anora.

If you pay attention in DAII you run into Harrowmount and Balon basically has the Cotiere or whatever out to kill Harrowmount.

Did all the good stuff.
Recruited everyone.
Romanced either Morrigan or Leliana or both.
Supported Bhelen in all but 1 playthrough.
Killed Branka and destroyed the anvil in all but one playthrough.
Killed Kolgrim and saved/preserved the ashes.Let Genitivi report about them and saved Arl Eamon.
Saved the dalish, cured the werewolves.Zathrian died.
Saved Connor and Isolde.Saved Redcliff.
Allistair marries Anora in all but 2 playthroughs.
In one Anora is queen alone.In another My male Cousland warrior married Anora and Allistair was kiled in the final battle.
Always do the dark ritual.
In one playthrough I did support Harromont and saved the anvil, supported Branka and killed Cairidin.

Hmm my first Orgins playthough.

Dalish Rogue

Theron Mahariel, stealthy rogue thief, assassin, duelist

Recruited everyone and did their quests

Went the Dalish and convinced Zathurian to break the curse

Went to the tower saved it

Went to Red Clif and saved the town, went back to tower and got Lyrium sent Morrigan into the fade killed the demon and saved Conor

"Killed" Flemeth 

Spared marjorie and didn't harden Leliana

Left Alistair sisters without giving her any sovereigns and hardened him

Killed Kolgrim, the dragon, passed the gauntlet and went through the fire naked and showed brother G the ashes

Saved Eamon, sent Jowan to the circle

Established the chantry, sent Dagna to the circle 

Sided with Cardin, killed Branka,  destroyed the anvil, made Harrowmont king meh

Oghren and Felsei

Made Alistair and Anora rule jointly after executing Loghain personally

Romanced Morrigan fully, did the dark ritual

Awakening 

Did everything

Destroyed the town and went back to the keep

Killed the mother, and left the Architect alive

Witch Hunt

Went through the Eluvian with Morrigan

 So long ago.... First attempt or completed game? My first attempt: Female Human Noble, Never recruited Leliana or Sten. Somehow got a partial romance with Alistair, and I made him sole ruler. I was dumped at the Landsmeet. I didn't harden anyone, and I didn't have the heart to make Alistair sleep with Morrigan. I was so hung up on sacrificing either Alistair or myself that I threw my controller in frustration and started over (mostly because I realized at that point I could recruit Leliana and Sten).  An epic fail on my part.
I actually completed the game on my second playthrough. I made sure to recruit everyone I could. (I didn't have Shale at that time) Where I saved the Circle, cured the werewolves, and elected Harrowmont. I completed the Alistiar romance, and failed to do most of the companion quests. I didn't harden anyone, and Alistair became a reluctant King with my character as a Queen. I managed to convince Alistair to do the ritual, and I got a fairly happy ending. 
By the time Awakening came out, I actually had played the same origin three or four times trying to make better choices.:D

Close.  The Harrowmount you run into in Kirkwall is a cousin.  The one who wants the throne has a first name that begins with a P - Pyral or Pryal (?) - and the one with the carta after him is Renville.

King--Harrowmout. Bhelen was relying more heavily on subterfuge. I supported Bhelen throughout, but gave the crown and promoted harrowmont at the last moment.  
I didn't support the chantry as a mage, but helped Dagna.
Anvil of the Void was destroyed.

Elves

I can't remember, honestly

Redcliffe

I know I sided with Jowan whenever and wherever possible, I was a mage myself. Killed isold, dealt with the desire demon to acquire bloodmagic.

Denerim

I know I reloaded when dealing with the bloodmage/slaver. I can't remember if I dealt with him first, and then reloaded and killed him, or the other way around. I suspect that I killed him, as I was an elf mage, but I'm not positive. 

Alistair was king, and got to bang morrigan. 

There's more, but I can't remember very well, and don't have the saves anymore.

Let's see what I can remember....I had no DLC installed at the time

Auriel Cousland, duel-wielding rogue

Missed both Leliana and Sten in Lothering

Saved Redcliffe
Sacrificed Isolde, then re-did the mission after saving the Circle so that no one died
Had Morrigan fight the demon in the Fade

Killed Wynne (took Morrigan with me into the tower)

Made Harrowmont king
Killed Branka, destroyed anvil

Preserved ashes, didn't bother with the dragon

I've always had Zathrian break the curse, but I ended up with the "killed the werewolves" achievement at some point

Replayed the Landsmeet several times so that my Warden could marry Alistair (unhardened, I think)

Fought Loghain, let Alistair kill him, imprisoned Anora

Convinced Al to do DR so we both survived

 I made it a point to remember what I did the first time, so that I would factor my initial-reactions to each decision in subsequent replays for my "canon" run. I only deviate if I find info I missed the first time that would make me choose differently.

Origin: Dalish elf, warrior.
Redcliffe: killed Connor.
Forrest: killed off the werewolves.
Orzammar (sp?): Bhelen for king, kept the 'anvil.
Circle: killed Wynne, recruited the Templars.
Urn of Ashes: Kolgrim attacks before making an offer (I was insolent).
Landsmeet: spared Loghain, crowned Anora, Alistair executed.
End: Dark Ritual, requested origin-specific boon, durr i dunno what i wanna do now.

I kept the origin for my canon. I switched to rogue, though, as it fit my playing-style better.

Retcons: cured the werewolves, sided w/ Kolgrim for the Reaver spec, Warden chose to travel at the after-party.

I felt obliged to side with Zathrian the first time since I was also a Dalish elf, but I missed the part where he said that it was only a temporary solution (whereas lifting the curse would be permanent), and enough of fellow Dalish fell victim to this sickness for me to feel strongly about ending it for good. This felt an appropriate retcon.

My word, that was so long ago but let me see if I can remember.
Oh, the main thing to remember is for my very first playthrough I did have the  "Equal Love" mod installed (Played DA:O on the PC), that being said:

Origin: Noble (male rogue; Arturo Cousland; romanced Alistair...and yes, the mod allows males to romance Alistair)
Circle of Magi: Saved the mages with the Litany of Adralla
Redcliffe: Connor and Isolde both survived; the mayor and barkeep did not.
The Brecillian Forest: Sided with the elves and the Grand Oak Tree
Orzammar: Sided with Harrowmount and Caridin. Later playthroughs I went with Bhelen.
The Landsmeet: Won the Landsmeet (only1 vote was cast for Loghain)  and beat Loghain 1 on 1. Killed Ser Cauthrian. Loghain executed by Alistair. 
Dark Ritual: Morrigan performed the ritual with the Warden.
The Battle for Denerim: Chose Alistair, Morrigan and Wynne to accompany the Warden. The Warden slew the archdemon.
End Game: Warden and Alistair married and ruled Ferelden (and once again, yes the mod allows that). The characters that could be hardened (Alistair, Leilana) were not.

One final thought: After the decision was made that Alistair and Arturo would marry and rule together, there's a scene where Alistair asks Arturo if he's really sure about the engagement. The first response was "I can think of worse things than being a queen."

My very first warden was a female city elf rogue and though she has changed a bit since that first playthrough, she is still my favourite.

The Circle
Spared the mages. Wynne recruited. 

Redcliffe
Managed to save everyone during the attack.
Freed Jowan and said she doesn't want to see him again (thus blood magic was not an option later).
Took a chance and brought the Circle to do the ritual. (I've since changed this to killing Connor for rp reasons.)

The Urn of Sacred Ashes
Pretended to take the deal with the cultists but didn't go through with it. 

Dwarves
Bhelan crowned. 
Sided with Caridin because he's a freakin' golem and Branka is insane. Anvil was destroyed. 
No chantry. 
Dagna joined the Circle. 

The Nature of the Beast 
Convinced Zathrian to lift the curse with trickery and threats of violence. 

Landsmeet
Loghain executed. (I hated Loghain and Anora in my first pt. They have grown on me.)
Hardened Alistair crowned. 

Companions/Other
Romanced Alistair (since changed to Zevran)
Did not do the dark ritual. (In later playthroughs I did - for this character, anyway - because I made her more pragmatic than she was in this pt.)
Alistair sacrificed himself :crying: but I reloaded and left him at the gate.

Dragon Age:  Origins was the first RPG I had ever played.  So I made a few boo-boos.  Here are the highlights.

Redcliffe

I went here first since Alistair recommended it.  I managed to save some of the village.  I sacrificed Isolde to fuel the blood magic ritual.  Connor was saved.  I saved Valena but I didn't know to go back to Owen to complete the quest.  (I didn't look at my journal much.)

Haven

Kolgrim killed.

I was playing on the PS3 at the time and my game must have glitched because I didn't have the brief cutscene showing a High Dragon in the area.  I just saw the gong by the path and being a curious sort...Oops.

Ashes were preserved.

Genitivi lived to spread the word.

Broken Circle

Mages saved.

Nature of the Beast

Zathrian performed the ritual.
Sided with the Grand Oak.

A Paragon of Her Kind

Harrowmont was made King.
The Anvil was destroyed.
Sent Dagna to the Circle.

Rescue the Queen

I chose to fight Ser Cauthrien and lost.  (Most big fights in my first playthrough were won by the skin of my teeth).

Landsmeet

I did not prepare for this at all.  I didn't talk to Anora at Arl Eamon's Estate and I didn't talk to the nobles in the tavern.  (I did rescue the tortured noble and got the ring from the templar, though.)  I ended up fighting it out with the nobility and Loghain's men right after having fought Cauthrien a second time.  :pinched:

When it came time to pick the new ruler of Ferelden I sort of lucked out.  I was playing a human female noble and was romancing Alistair.  When the option popped up to rule beside him as Queen, I grabbed it.

Loghain was executed.

Other Stuff

Alistair performed the Dark Ritual.
I had recruited Zevran but didn't get his approval high enough so he turned on me. 

The last member of my party still standing in the fight against the Archdemon was Dog.  So, the Archdemon was brought down by have its ankles gnawed on.

The good news is, I've gotten a bit better at RPGs since then.  :D

I really screwed up my first playthrough haha, but it was fun and a genuine experience. Definitely perfected my choices in future playthroughs.

Male Elf Mage;

Dwarves: Made Harrowmont King. Gave off the impression he would be best for Orzammar..boy was I wrong. Destroyed the Anvil.

Dalish Elves: Killed the Werewolves and kept Zathrian alive...yeah my biggest mistake.

Circle: Saved the circle.

Redcliff: Killed Ysolde in the blood ritual...I didn't do the cricle quest yet and didn't want to wait LOL

Companions; Never won Zevran or Oghren's respect. Only got them to "moderate inspiration"
-Romanced Morrigan
-Never hardened Alistair or Leliana...I had no idea such a mechanic existed

Misc: Didn't killed Flemeth, lied to Morrigan.

Urn of Sacred Ashes: walked through the fire with my armor on...had to kill the Guardian. Killed Brother Genitivi to protect the location of the ashes.

Landsmeet: Killed Loghain. Unhardened Alistair married Anora.

End game: Did the dark ritual with Morrigan. Asked for full autonomy for the Circle as my boon.

Definitely perfected and made up these desicions for future playthroughs. But this was my first uncut genuine desicions.

Over the various playthroughs I'm sure I've touched on every major choice in the game. But the first time playing DA, I remember taking forever to make *any* decision. When the Dark Ritual choice came up for the first time I just had to walk away...

Warden: Elven Tower Mage. And I think that's the only time I played that combination.

Romanced: Leliana.

Origin Story: Chose to help Jowan and Lily. Remember feeling guilty when it all went south and was prepared to turn myself over to the templars. Oh to be young and naive...

Ostigar: Trusted Morrigan and Flemeth about as far as I could drop-kick them. And Alistair managed to get on my bad side right off the bat too with the, "I hope you're not another mage..." comment. Loved the cutscenes with the battle with the darkspawn.

Lothering: Morrigan's ****ing really starting to get on my nerves. Recruited everyone, but.... I *almost* missed Sten. Hell, I thought he was just another prisoner. Just for the hell of it I talked to him on my way out of town.

Brecilian Forest: My first stop after Lothering. And really, really wished I had chosen someplace else. Got killed over and over and over... Those revenants just mowed through us like walking blenders on purée. But I did mange to save both the elves and the werewolves in the end.

Ambush: Killed Zevran!

Redcliffe: Okay... at this point I just wanted to stab Morrigan's eyes out. Redcliffe was the *last straw* - I never used her again in that playthrough. Saved the town, with some casualties. Helped everyone I could. Never found the kid (thought he was dead for sure...). Sacrificed Isolde to save Conner (didn't even consider getting help from the tower).

Urn of Sacred Ashes: Managed to kill the High Dragon on my first try (very proud...), saved the ashes, saved Eamon, etc, etc..

Circle Tower: Just two things stood out - recruited Wynne. Hated the Fade quest. Funny thing was, right before we went to confront Uldred there's this dialog you have with Wynne and Cullen. I can't even remember what I said, but seemed innocent enough at the time - and then Wynne suddenly goes homicidal on us! I was tempted to leave her dead for having these psychotic episodes, but I reloaded and carryed on.

Orzimmar: Recruited Oghren, sided with Harrowmont (one of the few times I actually did that), killed Branka (only once have I ever let her live), destroyed the anvil.
Denerim: I think this was the only playthrough I actually played cards with Isabella. :lol: 

Landsmeet: Won the Landsmeet, killed Loghain, hitched the (unhardened) Alistair to Anora.

Dark Ritual: DENIED. It actually took me a long time to come to that decision as I knew it was important. But in end my unreserved hate for the woman won out.

Ending: Heroic Sacrifice.
