Besides lag, not much.
Only thing that comes to mind now is idiots nuking spawns across the map during escort, pizza or 1234 objectives, but thankfully I´ve found this to be quite rare.

4. Ijits who won't use a missile to clear themselves a path to extraction zone during delivery. I turn on TC,grab the Obj, and bring it in. You lay there and think about why you're dead.

One of the players says, "Why'd you use a missile?"

No shock there. I just had a noob rage on me after a silver match (that he dropped into btw) because I was grabbing grenades.
He said "Thx for caring only about yourself"
I replied with 'thx for not using your missiles, bleeding out on wv 10 , leaving us alone to fight, and not doing extraction correctly. Learn how to play before critisizing others."
He then proceeded to tell me that he doesn't wastes missiles on Atlases when he has grenades, somehow implying that it was my fault he only camped at one supply box and the reason he died was because the box was empty. This is Dagger... boxes everywhere. On silver there are two nades per spawn. No excuse for camping one box when there's another 5 secs away.

The conversation went on for about 10 more replies before I finally said 'we're done'. Clearly he wasn't interested in playing better. How could I tell? He made comments like:
"I play for fun, not to do objs (objectives waves)"
"If my team is working then I'll use missiles" (implying from an previous response I sent to him that the reason he wouldn't use missiles is because I was running around map doing nade refills on my Slasher. Not realizing that if he actually moved around and helped the team that I wouldn't have HAD to keep replenish my grenades. Selfish players often can't see past their own faults and claim they are working as a 'team' by only standing there killing stuff).
"I've played plenty Platinums and they work as a team. Don't tell me how to play." (A response to yet another mail I sent him about how to properly extract on higher difficulties. I checked his N7 stats, added up all his Plat waves and it amounted to only 23 games. 50% of his games never made it to extraction. Quite the pro, yes?)

I was nice enough to suggest him to use BSN and read the guides to help him play better. He kept harping on the grenade thing. It just goes to remind me that BW created this dumb-downed version of a MP to appease ****s just like him. I won't let it get me down. There are other noobies who prefer to be good players. However they are becoming increasing difficult to find.

I get a lot of non-teamwork messages also.  It's always from a player who shouldn't be in the difficulty that they're playing.  The skill level of the players of this game has gotten to where teamwork is nearly irrelevant and if you're playing a difficulty higher than you can handle and wipe, you should just accept it and move on.  I never joined levels higher than I could handle before I was good enough at my current level because I never wanted to be a burden on anyone else.  Unfortunately, we don't all think that way.

While I commend your self-disicpline, the fault really lies with BW for not putting in a filter system to prevent access to higher difficults for unskilled players. I'd be a hypocrite if I said 'noobies shouldn't be allowed on 'Plat', because I used to farm plat back when the maps were camp friendly (and Pyros could be meleed ). By the time the map nerfs came, I was able to do Gold/Plat solidly and had decent enough gear for them. I never had a problem with carrying a noob or two, as long as they were willing to listen and not do really dumb stuff.

Things are quite different now than a year ago with bringing in AIU, Juggy and KroLord, noobs are able to play Gold/Plat with less difficulty. They'd rather drag out a failed game for 25-35 mins than to play a few silvers for creds.

Failure of basic math:
two 16min silvers = 70K credit (and maybe some lvl 1 consum)
one 25-30 min gold = 80K credit (cost you lvl 3-4 consum)
one plat game, failed on pizza delivery = priceless

^ While gating the difficulties may seem like a good idea, you have to consider how much that would fragment the playerbase and make it harder to find a game on Gold/Platinum. Perhaps the best way to do it would be basing it off extractions, rather than an arbitrary measure of "skill" (e.g. 10 Bronze extractions/5 Bronze solos to unlock Silver, 15 Silver extractions/3 Silver solos to unlock Gold, 25 Gold extractions/2 Gold solos to unlock Plat)

New pet peeve. Never run into this before but I've seen it three times in a row. Teammate grabs the pizza I run in front of him to start clearing a path. We've got two big clumps so I go ahead and rocket them and now it's a nice straight shot to the delivery zone. But when I turn around to make sure nothing is following my squaddie, I see he has gone the long way around, by himself, and walked straight into another spawn. This happened on Giant, Reactor and Dagger with three netirely different people. We came down to less than 30 seconds on one of those waves.

Right. I was merely generalizing 'unskilled' players (noobs), not promoting adding a skill tree system. The problem is a lack of a progression system as you stated (and I didn't properly clarify). They could've used your example or say lock difficulties until you unlock all X level guns/weapons (e.g. buying all uncommon guns and gear would unlock silver, and so forth). Alot of different things they could've tried.

Sure it would marginalize SOME of the playerbase but considering that most noobies that play higher difficulties are weekend warriors and most e-peeners do private lobbies you'd be looking at about the same marginalization. The reason guys like you and 88 can't find a good Plat pugs is because guys like me would rather play silver/gold than to deal with guys like them (noobies wrecking Plat). There are tons of guys I know who can play Plat but would rather play silver/gold to avoid the frustration. A progression system would encourage those guys to play the harder difficulty because they know they have similar experiece.

As far as soloing, that is just for people who need their ego stroked. :P

1) People who start games without a full lobby
2) People who start games without a full lobby and shouldn't be playing gold because you spawn in @ wave 1 with the person already dead and bled out.
3) People who start games early
4) Games that are started early
5) Session started, joining session in progress.
6) The person who doesn't belong in gold/plat playing host because they think that being hosst makes them safe from being kicked and allowed to play with no equip or gear with an almighty avenger --- who more than likely has also started the game early.
7) Joining a game in progress
8) Joining a game in progress at extraction
9) Waiting on other players... not because I cant play no no, because im joining yet another GD game in progress.
10) The host who starts early, then rage quits because they needed to be playing wood difficulty not gold/platinum.
