I know there are different choices etc. for this series and I have played through them a few times. However, has anyone seen or know of a diagram or chart that lists the playthrough for every mission/side-mission/dlc and what best order to do them in?

There is really no "best order." In fact, you should mix up the order between playthroughs for a very different experience. Doing Tuchanka: Bomb after the Coup, for instance, can be very different than doing it before. That said, here is my suggested order for missions to do between Horizon and the Collector Ship:

Edit: I give up, this commenting system's formatting is terrible. Adding color to text simply will not work. Just follow this the best you can...<_<

As for why I put the DLC where they are:
1. From Ashes- I know most people would say to do it ASAP, but I disagree. I put it after both Sur'Kesh and Grissom Academy because of how Javik can't be used right off if you get him ASAP and because of the Atlas Mech you fight in this DLC. It just pops up like any normal enemy, whereas the mech gets a big entrance in Sur'Kesh and a smaller but still significant entrance in Grissom Academy. This tells me that Sur'Kesh is meant to be the Atlas's introduction and Grissom Academy being the first occurance of it in a side mission, and thus both coming before Priority: Eden Prime

2. Leviathan- I simply like to take advantage of the fact that you can split the DLC up and play it in segments. So, I like to split it into thirds, each third ending on a combat mission. Plus, I think the various points of the story are not all meant to be in the beginning, middle, or end of the game. For example, I feel it is out of place for Dr. Bryson to first tell Hackett about his research right before endgame, and Shepard and crew will look like idiots on Thessia if they already knew about Leviathan because the entire DLC was finished early. One other thing, most people say to do the DLC right after Thessia, and I disagree because Citadel III, Thessia, and Horizon feel too interconnected for shepard to take a big detour to do things like this, Omega's DLC, or Citadel's DLC (as in, Shepard is completely obsessed with finding Cerberus after Kai Leng did what he did on Thessia, and it would be out of place to do anything but go to Horizon then and there)

3. Omega- I feel it is a great way to get revenge on Cerberus after pulling their little stunt on the Citadel in Citadel II. As for why I do it after the first third of Leviathan instead of before? It makes no difference either way, I just like taking care of the quicker one first [this case, 1/3rd of a smaller DLC versus the entirety of a larger one]

4. Citadel- I wait to do the mission part until after Rannoch because I want to bring as many people as I can to it, and it is after Rannoch that Tali officially joins the squad. I think it may be possible to bring her after the Geth Dreadnought [never tried it], but doing that means, similar to what I said above about Thessia, shoehorning in a huge mission in the middle of an interconnected plot arc. As for the Party, I want to have everyone be there who can be there, and you can't have Miranda until after Horizon. It is also the very last thing I like to do before the endgame.

I hope you found this helpful and sorry if it is a bit hard to read!

In fact, it's possible to bring Tali along on the Citadel DLC right after you meet with the quarians, without even doing the Dreadnought mission. However, I totally agree with the way you have it listed. There's an urgency to the quarian missions that makes it just feel wrong to take time out to enjoy yourself (or fight for your life, whatever) on shore leave.

From a meta-gaming perspective, I find doing the Citadel DLC best after Dreadnaught and Rannoch: Fighter Base. I want the Fighter Base mission for the two upgrades. Then the Citadel mission for the Suppressor and the mods. Then the arena stuff for more mods and the Cerberus Spirit Armor. Also, the claw game at the arcade has 4 weapon mods. It may not fit RP-wise, but it will buff your character quite a bit.

Also, I visit Bryson's office right after Priority: Palaven so I can get the mods there, most particularly the SMG stability mods. (Also, I think there is an SMG HVB mod there.) I have the collector's edition, so I have access to the Hurricane. But you need a stability mod to use it. The HVB will also make it much more effective.
