The fact that I could get approval ratings from Mhairi was a true frontal falcon punch to my face, with a chair.
I mean, seriously Bioware. Don't allow me to start becoming friends an awesome character and then proceed to kill her off. That's lame.
So yeah, I would like to bring Mhairi back.

Yavana, shame Alistair had to kill her she was cool and mysterious. I wish that she actually had a chance to use Maric's blood to awaken the dragons.

Duncan, at the very top of my list is Duncan. He wasa good and honorable man in all ways, I would love to have seen more of him.
In addition there is Mhairi, by far the best companion in Awakening, but you lost her early on.
Finally in my playthrough Alistair always ends up dead.

DA:A Anders, he was cool and comical. Bethany if Hawke was a mage, I kind of wanted a big family in DA2! Grace, she was hot. And I would also bring back Mhari, she didn't deserve to die.

Justice. And I mean the real Justice; none of this "appear in one cutscene as a twisted shell of itself and then become nothing more than a gameplay mechanic" nonsense.
Admittedly, I've only had the will to stomach DA II once, so perhaps I'm not remembering if Justice appears in another instance besides the Chantry, but my original point of him being largely an afterthought still stands.

The Fade is a big one, since Justice takes over for that whole time. Also, during one of Anders' quests where you have to pretty much hold Vengence back from killing a kid. You see it very briefly when it starts the fade to black scene with a romanced Hawke. Oh and if you rival Anders, Justice/Vengence will come out from time to time to chew you out. Leaving Anders midly curious as to why Hawke suddenly looks a little horrified and creeped out. And, of course, you see bits of Justice at the end, when the Chantry is about to do a great Marvin the Martian impression.
