Wondering if anyone else has ever experienced this.  I'm playing on a modded PC ultimate edition.  I've played through the dwarf noble origin twice before and didn't encounter this.

When I was captured, my hide helmet inventory item disappeared with the rest of my inventory but I didn't notice until I was already in my cell talking to Gorim.  At that point it's one cutscene after another until you're sealed in the deep roads.  I reloaded to get screenshots without the helmet and couldn't get the locked-in cutscene to replay no matter what I did.  In addition, my PC materialized in bizarre DLC armor that I must insist I would never put on her voluntarily.  With no locked-in cutscene the exile quest wouldn't progress.  I was able to get around it by a weird map glitch.

Here's what I did.  Walking away from the door I should have gone through, the hall comes to a T intersection.  Both branches come to other T intersections with all terminations ending in blank black walls.  When I walked into one, I expected to keep walking in place.  I kept going off the map.  Mapped aread showed up on the main screen as orange blocked areas laid out where they should be.  The minimap showed where I was in relation to the mapped areas.  I could walk wherever but if I passed through the wall into a mapped area I was in the rendered version.  Once I walked into the middle of a rock wall barricade set in just for the origin story.

After all that, to get out I walked around (to the south of) the door I couldn't get past, travelling west until I bumped into the first encounter area.  I walked through the wall, fought the spider and the game progressed normally from there.

Has anyone encountered anything similar and what did you do to fix it?

I have not seen that, but I will start a noble origin character in the morning to see if it happens.

Don't drive yourself nuts!  It only triggered this time because I was doing screenshots and reloaded after the first screenshots with the helmet.  If I did something else differently, like trying to pause in the middle of a reload (which I've done but don't remember doing this time) it can scramble the save game info.

I think you're right.  I usually remember to take the helmet off so the hide helmet mod isn't in effect when the dwarf noble loses it's inventory.  I forgot this time and when I went to reload I think I hit a corrupted save.  I was lucky they just reused a larger layout that was accessible the way it was.

I finally found out where I saw reference to this bug. It occurs when a save is created while script is still running. In my case, since I was saving and replaying for screenshots, I was using an incomplete save that I made right before I went through the hole in the wall.

Finding a save before the bad one happened is a better workaround but if there are half a dozen named saves to choose from it doesn't hurt to just try walking into the black area and through walls.
