I will be replaying DA:O soon. I've beaten it a couple of times on nightmare difficutly, and will be looking for ways to make this play through a bit more challenging. What are some of the rules you make up for yourself to make the game more challenging? No potions for example or never purchase gear. I'm not interested in trying a solo playthrough, as the companions are my favorite part of the game. I am interested in hearing your thoughts or any recommended mods for this.

If you want a new challenge, you could try beating the game at the lowest possible level, or try 'DA:O done quick'.
Both involve going straight for the objective and ignoring all distractions, and in both you'll have to make do with poorly trained characters and poor(ish) equipment. Both involve careful research and planning of missions, and experimenting with new tactics/strategies. They both give a unique, fresh perspective to all missions.
My rogue (Pythia) reached Ostagar at level 2 (just barely) and made it through the Joining still at level 3 (just barely). She is currently doing recon and training for the assault on the tower.
A useful supplementary rule would be "no 'injuries'", i.e. no feeding companions to the wolves.
There is some synergy between the two styles, and research for the MinXP run should be of great help for 'DA:O done quick'. However, the objectives are sufficiently different to result in a different outlook and different approaches. E.g. Attack at Nightfall in Redcliffe: for MinXP you might heal and protect allies so that you can feed them pre-weakened enemies for XP-free disposal, while a Sith Lord doing a DAODQ run could simply burn up everyone, allies included. Or simply let matters resolve themselves by spending the night elsewere.
