This sounds very much like a tweak called the No Helmet mod. I suspect at least one of your mods includes it. It basically means helmets only appear (automoatically) during combat and disappear for anything else. Which eliminates those jarring cutscenes when helmets appear and disappear mid conversation.
Personally, I find it just as immersion-breaking with helmets appearing just for combat. That's why I don't equip helmets at all. Which I realise is me hamstringing myself, but I'll take it for the sake of immersion.

EDIT: It seems you were right, while installing other mods for auto-loot functions, i downloaded a package that also hides helmets. Apparently the mod remembers helmets, to equip them in combat. Though for whatever reason, it was forgetting to equip them once combat had started. Problem solved!

Just as a heads up, you might be interested in the no helmet hack which does a much better job of it.

I do the exact same thing for the exact same reason (I'm aware of the no helmet hack that dainbramage mentioned but I found it caused issues for me). I do keep one in my inventory but I rarely equip it because for your average trash mobs or Lt. level enemies the loss of armor and stats is negligible. I do equip one for boss fights though (but only if I'm playing a melee character).
