I think the guide might be off/patch changed the conditions because I saved the Heretic base in ME2 and was able to achieve peace between the Geth & Quarians.

Outcomes:
1. Destroy Geth Heretics and have Tali exiled(Admirals cannot know of Tali's father's experiments, so you must lie). Conflict between Legion and Tali must be resolved, and both must survive the Suicide Mission.
2. Rewrite Geth Heretics and convinced Admirals that Tali is not guilty(Cannot present Tali's father's experiments). Conflict between Legion and Tali must resolved, and both must survive the Suicide Mission.
3+. You CANNOT mix these around, it must go in this order or else peace is not possible.

Here is the wiki's take on achieving peace:
Note: The Reputation check's requirements are complex. First, there are factors which seem to be purely pass/fail; missing even one of them flunks the Reputation check:
Shepard must have at least four bars of Reputation.
Tali and Legion must both be present, which requires an imported save from Mass Effect 2. If no save was imported, Tali will have been exiled and will not be able to support Shepard with an Admiral's authority, while Legion will not appear at all.
The mission Rannoch: Geth Fighter Squadrons must be completed.
Additionally, there are other factors which build up Shepard's trustworthiness to both parties; if Shepard has not done enough of them, the Commander will not have the groundswell of goodwill needed to moderate a cease-fire. According to the official Prima Games guide, each factor has a certain "point" value, and at least 5 "points" are necessary for the cease-fire to occur. These factors are:
(+2 points) Destroying the heretic geth in Legion: A House Divided. In shipboard conversation, you can ask Legion about the repercussions of your decision, and it will mention that the absence of the Heretics made the consensus to join the "Old Machines" more difficult to achieve. Geth forces are also considerably weakened without the aid of the Heretics, giving the Migrant Fleet an edge in the battle and reducing the quarians' overall casualties.
(+2 points) Preventing Tali's exile in Tali: Treason without presenting evidence of Admiral Rael'Zorah's experiments on the geth. While it's possible to present the evidence and still be able to broker peace, it adds its own problem in that it makes Tali far less likely to survive the attack on the Collector Base.
(+1 point) Brokering a peace in the Tali/Legion loyalty argument. If the player did not have sufficient Paragon/Renegade points at that time, loyalty of either Tali or Legion would have been lost. In that case -- even if loyalty had been regained later -- this "point" will not count towards the five needed to pass the Reputation check (and broker the ceasefire).
(+1 point) Completing Rannoch: Admiral Koris.
(+1 point) Saving Admiral Koris during Rannoch: Admiral Koris.

+1 for completeing the Geth consensus mission
also you mentioned Admiral Koris twice +1 for saving Koris and if you choose not to save him you loose the point regardless if you do the mission or not

+1 for completeing the Geth consensus mission
The wiki indicates the the consensus mission (listed as "Rannoch: Geth Fighter Squadrons") is mandatory. I suspect this is the case, otherwise the dialogue about what Shepard saw while in the consensus would not make sense.

I would check out the wiki for the conditions needed for peace between the Quarians and Geth. They are quite involved. You can rewrite the heretics (it is not recommended) and still achieve peace, but the window becomes narrower and you need to do everything else right.
Here is the wiki's take on achieving peace:
(+2 points) Destroying the heretic geth in Legion: A House Divided. In shipboard conversation, you can ask Legion about the repercussions of your decision, and it will mention that the absence of the Heretics made the consensus to join the "Old Machines" more difficult to achieve. I always tought that this decision didn't mattered... Only in the War Asset Count:
Destroying Heretics(+ for the Migrant Fleet - for the Geth Fleet)
Re-Writing Heretics(+For the Geth - for the Migrant Fleet)

-Tali and Legion must survive ME2 (requires "import")
-Shepard must enter Geth Consensus (ie complete Geth Fighter Squadron mission).
-Shepard must have 4 bars of rep
-You need 5 of the following points if Paragon, and 7 if Renegade:
- Destroy Geth Heretics in ME2 - 2
- Tali not exiled - 2
- Resolve Tali and Legion conflict with charm/intimidate - 1
- Complete "Admiral Koris" mission - 1
- Save Admiral Koris at the end of his mission - 1
I think the above is accurate, but have never tested it in great detail. It has however been reported slightly differently on the forum, where the consensus mission is not required and is worth 1pt, and you only get a point for Koris if he is alive (not for just doing his mission). In any event, if you meet the ME2 requirements and do both Rannoch side missions, you are likely going to be able to pass the check.

And I have 1 last question. I'm finishing up My squad LM's [Tali's , & Samara's] in ME2, and I was wondering if I can do Tali's LM after going to the "Derelict Reaper" so I can take Legion on Her LM. If I do that, will I be able to Complete [Tali's , & Legion's] LM befor the Collector's abduct My Crew? Any help would be appreciated.

Once you wake him up, you can go right ahead and do Tali's.

Yes. See the above linked thread for lots of detailed discusions of when the kidnapping occurs, how to have Legion available for various missions, and other related topics.
Also, it is quite possible to have a number of non-loyal squad members survive the Suicide Mission. So it is not critical that every single loyalty mission be done before hand.

Yes, but make sure to wake up Legion before you go and do Tali's, because you need to do a mission for Legion to give you his. If you don't wake him up, and you do Tali's, and then you wake him up, it'll go right to the abduction.

Ventilation job:
-Tali
-Kasumi
-Legion
Second firing team:
-Garrus
-Miranda
-Jacob
--------------------
Biotic bubble person:
-Samara/Morinth
-Jack
Second firing team:
-Garrus
-Jacob
-Miranda(Can be unloyal, and will not die)
Escort for the captured Normandy Crew:
-Anyone, but Mordin is recommended since he has a very high chance of dying(Anyone loyal can do this).
-----------------------
Squadmates for the final run:
-Anyone who is loyal, but it's best to bring anyone but Grunt. Grunt is very valuable for the team that will be defending against the Collectors. It's best to bring anyone else, and if you bring Miranda she does have a special dialogue.

After the crew gets captured, you cannot enter any HUB or it'll result in the death of half off your Normandy SR-2 crew. The only HUB you can enter is the Shadow Broker base which seems to be the only base you can enter and not affect anything else.
Incorrect. I have travelled to all hubs and still had all the crew survive. You just need to not get any Mission Complete screen and you will be fine.
