Cunning, unique personality, deep character, appealing look of a villian, bad ass dialogue and cool name.
Irenicus. Best. Villian. Ever.
He was great because he was and had all of those things. And as much as you wanted to just kick him straight to the Void, he was also a sympathetic character. He had a sister for whom he held affection, a lost love that he desperately tried to remember and feel again even when he was losing his grip on his humanity/elven-ness(?), and a loss that deeply changed him and fueled the revenge that made him so detestable. Pure inspiration.
Oh, but you left out a voice actor that brings it all to life. Thank you, David Warner.

I think a villain should be, first and foremost, entertaining.
Look at The Master from the first season of Buffy the Vampire Slayer- every scene with him kept me hooked. He was scary, witty, undeniably evil, but he was also just a little affable. He's the one I remember the most out of the plethora of villains that that show had, and it's just too bad that he wasn't around for very long.
A good villain makes me care about whether the heroes can defeat him, and the same goes for the heroes.

I personally believe the best villains are dark reflections of the hero like say a Warden that follows their duty to the extreme.

To me it seems like Flemeth is bigger picture. She has changed the fate of maric and crew, warden, hawke, maybe even the inquisitor.
It seems as if she manipulated morrigan, but now she's exceeded her expectation

A great villain does NOT monologue when he or she has captured the hero. That hero should be dead within about two minutes of capture.

I don't understand why people consider Loghain as anti-villain.
I like villians like Kreia (Darth Traya) a manipulative mastermind who is alone pursuing the only moral agenda in galaxy. A villain who makes you question her motives and believes. The best game that IMO handled that is Legacy of Kain series.
Kain and Raziel are the best of case what defines anti-villain and anti-hero.

Depends on the villain. My favorite villains aren't so much villains as they are people circumstance has pit you against. You have your agenda, they have theirs.
For any type of villain, one that is actually competent.

Later content (like Return to Ostagar) and Loghain's words if you make him a Warden seem to have taken strides to justify what he did. Personally, I think the claims and his actions were entirely at odds, but some people chose to believe them.

I think what makes a great villain, is expressing a personal belief that you could understand. You don't have to agree with it of course, but you can understand the reasoning behind it.

If they are fighting a hero on a moving platform, and see the hero duck, they should duck as well and not just turn around and see why the hero ducked in the first place.

The Hero should be promptly killed if captured, no last requests or lengthy questioning sessions.

The villain should also see the corpse and not just leave the hero in some "fool-proof escape-proof" death trap and assume he's dead.
