Well, I usually don't replay games for X number of times in a row. Doing so would get boring fast. I notice that when I game, especially if it's an RPG's OC, I play it once and leave it for about a year and come back (effectively making sure that the next experience is a 'new' one all over again). Of course, for modules associated with the game (like NWN, NWN2 and DA:O ) I have come to expect that at least most of them would have their own custom soundtracks to spice up the atmosphere.
On the other hand, I used to change the background music for Sims 2 sometimes. But that was easier to justify since I played Sims 2 so many times until I was bored of the jingly little themes, and it was a game about real life, after all. Plus there were some amazing tunes I couldn't pass up; I once used Paul McCartney's "Peace In The Neighborhood" for the background song. And it also made sense that some of my more colorful Sim characters would love to have things like Tupac's "Changes" or Aerosmith's "Dream On", or even Avril Lavigne playing on their in-game household radio.

While playing arcade games, I usually go with the mellow psyche rock like 10 Years, if I want something heavier maybe Chevelle.
With the Sims I love really mellow stuff like Lisa Hannigan, Loreena McKennitt, or Anna Nalick and Bonnie McKee.

Sadly, my Sims are more inclined to be wannabe rappers or rock stars. I get a kick out of imaging Sims happily rebelling against the suburban idyllism and conformity that Sim-world represents, and in the true tradition of anarchic Sim-dom what sane Sim wouldn't want to find a reason to frazzle the parents, irritate the neighbors, etc etc? Ha, my Sims are almost all recluses. My girl sits on her computer writing novels while her wife makes airplane noises in front of the mirror. The mellower, the better.

If I do, though... well, I like metal, especially power metal. Helloween, Blind Guardian, Dio, Judas Priest, Gamma Ray, Rainbow, Demons & Wizards, Domine, Blue Oyster Cult, Iron Maiden, Rhapsody of Fire, Black Sabbath (usually Black Sabbath with Dio), that kind of thing. Sometimes Megadeth, or Queen, or Alice Cooper, or Hawkwind. What exactly depends on the character I'm playing.

post rock stuff like Godspeed You! Black Emperor, do make say think, mono, mogwai...but for the most part the game music is fine for me
