I was going for the insanity achievement. I just completed the SM and nothing happened. When Shepard first woke up in the Lazarus lab the difficulty was set to veteran, but then I changed it to insanity and never changed it after that. Don't tell me I had to set the difficulty before even starting! I read that the difficulty needed to be set before Freedom's Progress.
What happened to my achievement????

From what I remember it has to set to insanity at the very beginning during the character creation part when it brings up the settings you have.

Crap. I just started another game to test this out and the first thing it gave me was the game options menu. I don't remember that the last time. That is why I changed it in the lab and not before starting the game.
