I was playing on the citadel, i just got done talking to cortez in the docking bay, and I looked down, i quickly looked back up when i noticed shepard had glitched and was falling through the map.
I thought the game was going to freeze, but he landed in the refugee camp about 30 seconds later.
doubt i could recreate it.

So has anyone fallen through the citadel?

I've fallen throught the Citadel but it was because I was talking to someone and skipped through the dialogue. I'm starting to think this game was not carefully tested before launch..

What I saw someone else say was that they fell out of Huerta Memorial and landed in the Presidium Commons - what this tells me is that it would have been entirely possible to go anywhere on the Citadel without loading screens were it not for the resource limitations of consoles.

Nope, ME2 wasnt either. Think of it back to ME1 days and it also had plenty of glitches. But ME2 and ME3 are quite the offenders especially when coming up with weird or just off the wall glitches. Like in ME2 in several of the missions where the aircar is used I see the aircar ghosting across the screen. Then there is the issue of npc headtextures sometimes partially displayed with a big emtpy place in the middle of the forehead.

Using freecam on the pc will show you how much is loaded up at one time; but clearly if it wasn't for the fact it had to cater for the consoles and was just a pc game then there would be no loading at all.

I like the glitch you get in liaras office where you both track glyph rather than look at each other when talking.
Had one last night where James and garrus are talking in the kitchen and shepards head just rotated through 360 degrees constantly. I grabbed a screenshot I'll upload later, it was most amusing.

I was in the docking bay and that moving keeper managed to knock me out of the area with a walk path and I had to reload. I also did once fall off of Legion's walkways in the geth consensus.

I had a glitch on Omega where the floor wouldn't spawn after opened the door after I took an elevator up. I kept falling down. I had to replay the mission to undo the bug. Stupid Omega bugs and Bioware bad polishing!

^Had a similar one on Omega fighting the last sequence. Saw an open door to leave and went though and did the eternal fall thing...critical mission failure. And then there's the Kai Leng Thessia fight that ends with him convulsing on the ground after lashing him over and over - I actually like that one since he's such a pathetic boss. Those types of glitches are kind of humorous. It's the repeating ones - the ones they've had a year or two to fix that kind of annoy me.
