i played this game 1000s times, but my question is, must you chooose all heart options to have a romance, or will it start with only choosing 1 or two of those dialog choices.

Not all heart options are mandatory for a romance (specifically the act 1 ones). However, if you want to play it safe, pick them all.

Can you have a romance with more than one character, I wonder.

Yes and no. You can sleep with more than one character, but you can only maintain one relationship in the end game.
