Morrigan did hint that the world she was travelling to was pretty dangerous. Maybe he died defending his lady love.
There are plenty of examples however I would have thought that the strongest evidence would be actually SEEING the Warden walk through the Eluvian, I mean if this isnt evidence enough to suggest that the warden might not currently be in Thedas I have no idea what is.Loghain Mac-Tir wrote...

Because there is NOTHING (note the emphasis on 'nothing') to even remotely suggest that Hawke had gone through the eluvian in DA II. More over I am not sure Hawke even knows that the Eluvian can be used as a passage way, did Merril ever tell him that, does she even herself know....
