And the whole Joker disease thing... how exactly did Batman get that out of his body? Was this also just all fake plot development created by fear-toxin or did Batman really just remove this by overcoming it mentally? That is weird, but I thought the whole "Joker's Disease" plot-point was nonsense to begin with so it's fitting it was also resolved with nonsense.

Joker's apparition was a result of the new fear toxin. It's not far fetched to assume Batman had overcome it given he managed to overcome a dose strong enough "to drive 10 men insane" in Arkham Asylum. By the time of Arkham Knight he's been dealing with Scarecrow for years, along with his fear toxin to not let it beat him. He beats the apparition by locking it deep inside the back of his consciousness, which I thought was well enough explained, all things considered.

What injection? He wasn't injected with the fear toxin -- The Joker already existed inside Batman ever since he died and even before so. The toxin simply provoked that particular fear even further.

The fear toxin isn't a poison in the same way as that of the Titan poison which one can't just shrug off. Batman has battled Scarecrow and his toxin for years and the training he's been doing while traveling the world before eventually returning to Gotham and taking up the cowl has given him an almost indomitable will to fight it.

 

It's hard to start complaining about realism when Lazarus pits has been an established 'thing' in the comics for years, along with biological weapons that turns people into mutated brutes.

While I have not played AK, I have looked up some stuff regarding Scarecrow in the game so I'll just put this in spoiler tags.

Well my real issue with the whole thing is that Scarecrow has his own toxin used on him, and he reacts similarly to how he did in Batman Begins. Since he created the toxin (and as it was in the comics) he would end up developing an immunity to it, especially if it can be used in an airborne form. In Asylum he did have the gas mask attached to his mask, and while it's still present in Knight it's quite obvious that it isn't really covering his face which kind of defeats the point of having it in the first place.

I'll reserve judgement until I play the game. I'm not sure when that'll be since I'm on PC.  

 

I can say I wasn't impressed by that intro. I preferred the intro to Origins actually.
