In DA2 on the companion inventory screen, under their armor is 4 bars (right now Bethany has 1 of those bars filled). What does these bars represent and how do I get all 4 off them filled up?

That would be the famous follower customization in DA2. You find items here and there, in the game, and these gets added to the follower`s "armour" automatically. You can tell it is there when one of the bars in the character`s inventory screen has been filled.
