For the past few hours i have been doing some searching on how to do all 3 of these things, the best i could do was change the appearance and voice, but now i have found away to do them all quite easily.
Of cause this involves the dragon age toolset and this little program

I am going to use the example of my character,
My old character was named Jamie and was a human male with the wise voice.
I changed the apperance voice gender and name to
Emiko Human Femal with the mystic voice.
These changes will effect Your Head Morph (face appearance) Your Voice (not that you hear it much) Gender (Your body shape (female or male and how in game npc reacts to your gender (will call you a lady instead of man after change) and name (your name).
Each 4 of these things can either be done seperately or together.
Firstly Appearance what you want to do is go into the game and start a new game and create the character you want, (my only rule is it has to be of same race and class at the moment) (same class since you cant have a dwarf mage :s racist i know lol)
So lets go with you want your character to be a Human Female.
You make her, glam her up and all that, mods can be used for different hair styles and the such if you must.
After you finish making her, start the game and get to a point where you can save.
Next follow the instructions of that program in the link above.
After you have done so and is successful open up dragon age toolset. (you may have to apply a 4gb hack, this can be done by a program called CCL Explorer) to get the toolset working properly.
Once opened go to File>OpenFile>My/Documents>Bioware>Dragon Age>Characters>(OldCharacterName)>Saves>(which ever slot or autosave you chose to modify with the above program)>OldCharacterName.das
Open that up.
First thin you want to do is if you changed genders is also change the coding for the body. My old character was male and i want to switch it to female. To do this we go
SAVEGAME_PLAYERCHAR
SAVEGAME_PLAYERCHAR_CHAR
SAVEGAME_APPEARANCE
SAVEGAME_APPEARANCE_GENDER - Change the Value of this from 1 (which is human male) to 2 (Human Female)
That will change the body to a female and how other npc reconize you as. (options 1 and 2 are for human races only, different options for elfs and dawarfs, you are gonna have to figure those out for yourself, best way is to open the new character save you created and look at the values for the same lines)
Next is Voice - i had a male wise voice and want a female mystic voice
Under SAVEGAME_PLAYERCHAR_CHAR
SAVEGAME_PLAYER_SOUNDSET-------
----- Both of these have to be changed from ss_pc_mhuman_wise to ss_pc_fhuman_mystic
SAVEGAME_DEFAULT_SOUNDSET------
There are other voices which are named in when creating the character.
For Name
Under SAVEGAME_PLAYERCHAR_CHAR
SAVEGAME_OBJECT_NAME - I had this as my name 215786:jamie (numbers may be random) and changed the value to 215786:Emiko
After that click the save button (the toolset makes a backup of the orignal files with .bak on the file extention) and exit this file then open up the file (Oldcharactername).das.met
Change
SAVEGAME_META_GENDER - value from 1 to 2 (human male to human female)
SAVEGAME_META_NAME - from jamie to emiko (what ever name you had to what ever one you want)
SAVEGAME_META_SAVENAME from jamie to emiko (what ever name you had to what ever one you want)
Save the file and exit, boot up the game and load the save file, and surprise, you just had a sex change 
You can change your class and race also, it is possible under the same instructions as above but you got to make some more in depth changes such as to skills and tallents, levels and all.
Sorry for the long Tutorial, but i hope this saved you time in trying to figure out how to change your gender completely mostly.
