I always need a few seconds to adjust after I've just used Acolyte, Javelin or Venom for the game before... just visualize running with the Harrier and shooting in all directions..LOL!
I usually avoid playing AIU after I'd just played Paladin because I'd most likely get down twice or more using the wrong button.
I don't see what's wrong with bringing a lvl1 character to gold. As long as you have a good load out and weapon you're good to go.
Missing an extraction on Dagger that could have easily been made, because I'd forgotten that there were two possible extraction zones, didn't check, and made the last-minute dash to the wrong one.
I've done that several times, including once on a gold solo. Didn't extract!
While I tend to be scrubby most of the time, my scrubbiest moments usually come when switching to a different character and forgetting they've got a particular power until wave 4 or later. Happens a lot when I switch to a character with tactical scan, though happens with other powers, too. Absolute scrubbiest was when I didn't know overheated weapons (PPR, CSMG) come back if you just walk (not run) around. Went to end of entire waves gun-less.

I joined a bronze game, turned out to be the end of wave 10. At the beginning of wave 11 host disconnected, and apparently every one else but me, so I was soloing bronze wave 11. No problem, right?
Anyway I went around the map a bit and at about 20 seconds in timer I went to extract zone. Turned out to be a bit too early. The reapers (I think) threw nades at me. I moved a bit of course but missed one. Blam, right under me. About 2 seconds in timer, medigel too slow, oops, mission failed :Z
Today my Phoenix Adept took the long ladder on FB Vancouver --- just to lose his grip on the last rung. He fell down and had to climb up again.
And failing at Nova canceling while dealing with Possesed Abominations.

I am so guilty of this. I accidentally did that last night in an otherwise super smooth game. I was host too so I now declare myself the scrubbiest host of games...maybe even parties.
Once I hit a target in the chest instead of the face.... darn console auto-aim.
Spending at least 10 minutes taking out the last two Atlas in a Cerb/Gold/Godess when I was clutching wave 10 as :

Getting downed by a Swarmer certainly takes the cake, lately. I can have Primes and Banshee shooting the crap out of me, but what enemy gets to take me down eventually...a freakin' Swarmer 
I almost forgot. Platinum, Hydra. I'd basically soloed the entire thing since my team was a collection of the worst possible kind of noobs (the ones with the high N7 and challenge ratings that trick you into thinking they're actually worth something) I carried them all the way to wave 10. All targets had been taken out...by me...and one Banshee is the only enemy left on the map. I get her down to 1 bar of armor aaaaaaaaaaaaaaaaaaaaaand, you guessed it. Ghost hands insta-killed.

I've got about 666 hours of scrub under my belt. OK, well, more like 666 hours, 50 minutes. I have about 3 minutes of awesome.

i've been constantly forgetting about the barriers phantoms, banshees and praetorians put up when a projectile power is targeting them. serves me right for not playing for about 8 months or so lol
