It is possible for a party member to end up hating the PC if approval gets low enough, yes, and it can come to a crisis point with that character-- at which point they may even leave for good. Or other things.
I'm with you on that, I'd like them to react to my choices and if they don't like them then leave, also i'm interested in what other thing's they may do based on everything we do in DAI and i'm happy about the direction they seem to be heading and some thing's that could happen.
It'd be great if they left you and began to actively work against what you are doing.
Weren't Bethany and Carver mutually exclusive in a way? Depending on the class you choose anyway. That is, choosing a mage will give you carver, and choosing anything else will give you Bethany. And from what I remember, in the climax, Sebastian and Anders were mutually exclusive. If you killed Anders, Sebastian fought alongside you (even if you sided with the mages). If you spared Anders, Sebastian left your party (or so I recall).

I think that DA 2 showed the two ways in which the game could present you with mutually exclusive companions:
1.) The decision is based on character build (like Carver/Bethany, Handmaiden/Disciple, or Hanharr/Mira)
or
2.) The decision is based on player choice forced by a plot point (like Anders/Sebastian, Alistair/Loghain, Kaidan/Ashley, or Dynaheir/Edwin)
I prefer the second over the first, but both add replayability.
