Does it count if you've done it, then gone, "Nope!" -> restart mission? Or does it only count if that's the decision you carried to the end? 'Cause if it's the former, then I've done almost everything. Except several of the romances, or kill Wrex in ME3.

Ha. That sounds like my games, almost. I once sided with the Geth, pretty much by accident. Didn't restart though. Went back to the Normandy to check on everyon'e post-mission dialogue. Ran one or two missions after that, then dropped the game. Same with Tali. I had her die in ME2, played through the Rannoch arc, then dropped the game.
I have never romanced Garrus, Cortez or Jacob.
I have never killed Wrex in ME3. (If I want a run where I will kill Mordin/Wiks, I make sure Wrex is not recruited or dies on Virmire.)
I have never died during the SM. (Umm, I did all this work and now I can't import!?!)
Probably some others.

@CrutchCricket, Dominate kinda sucked in ME2 didn't it? Seemed that way to me. Anyone you targeted with it fell over the second you cast it on them, and by the time they got back up it was already wearing off...

I always used it on my Bastion Adept so with all the upgrades it could last quite a while. And for my purposes it didn't need to last longer than 10 seconds or so. Target a basic mook (or a rocket or Pyro when applicable) next to a bunch of other mooks. Then when they all turn on their former compatriot me and the team move in close with shotguns and biotics to unleash hell (or just shoot at them safely from a distance while the chaos has them distracted).
Plus, against Collectors if you time it right you can possess a Drone right as it turns into Harbinger and then the leader of genetic destiny kills his own guys.

This is actually harder then it seems for me...
Never imported a Mass Effect 3 character into Mass Effect 3(unless you count doing the prologue so I can upgrade guns to level 10 for achievement)

Never played the same story twice(every trilogy run I ever do or will ever do in the future will have something different from the one before it)
Never romanced Kaidan(as male) or Steve, possibly add Jacob to that I can't recall doing him or not.

I know. I saw a clip on Youtube that didn't take the interrupt and it was kind of disappointing how similar they were. They should have rewarded people who didn't take it by being able to equip the sword for melee attacks. BioWare pls!
