I don't think I have ever failed that persuassion check. I have failed Tali and Legion's on multiple occasions though.

I only failed the persuasion once because I did it on purpose to see the outcome, which later I was able to rectify, otherwise I always get both their loyalty when siding with Miranda.
I never had Legion go in the vent during the suicide mission.

Most of my playthroughs I hand it over to Cerberus.

Both have their points. But I have to threaten them or tell them to take it outside, later.
I take the blue option with Tali/Legion because I agree with it. Your war is dumb, quit it. For all the good that does...
I've never worn any headgear in me2 or 3 apart from when the game makes you.
No idea why.
I usually opt for the headgear that isn't helmets, mostly because talking to people with them on is a suspension issue for me. Stuff like goggles and visors.

I never put FemShep in the Securitel helmet. Looks great on MShep (wish it wasn't as shiny) but on FemShep it just looks... weird.
I never use the Collector, Blood Dragon, or Terminus armor, "Kai Leng" visor or capacitor helmet.
I always love it when people slap Shepard with the helmet on (ouch!) or somehow instantly recognize him from his appearance in armor.
I do love the Blood Dragon armor in ME2 (hate it in ME3 for some reason) and I used the Terminus Armor on my melee Sentinel for a long time because it looks cool with the Tech Armor. I actually like the Kai Leng visor on FemShep, which is again odd because I loathe that character.

I'm currently doing a "kill all male squadmate" playthrough which will be followed by a "kill all female squadmate" playthrough and then possibly do the above playthroughs I never have done.

That's what I replaced it with once I got all the pieces. Spray painted it that shiny goldish-green color (I'm not exactly sure since I'm somewhat colorblind).

I can't mix and match in this game. A lot of the pieces were designed to go specifically with the rest of the pieces in the set and it bothers me to jumble them.
I've never picked Ashley over Kaidan in ME1. Thinking I might when I get to Virmire in my current playthrough but it's gonna hurt.
The Securitel helmet looks dumb. Like special needs dumb. In my circles it's known as the safety helmet. Yes, in that sense.
I wish we could set different helmets as default. I like the Death Mask for its bounty hunter-ish tendencies but I still want to see Shepard's face and it gets annoying having to remember which missions require a helmet and manually changing every time. And the "off in conversation mode" setting is horribly bugged. I remember the first time I tried it I had to redo practically the entire Sanctuary mission because pop-out-eyes-Shepard ruined it for me.
On the topic of fashion I've never:
-worn non-custom armor sets
-stuck with default N7 armor
-wore any visor or helmet that wasn't the Death Mask or default because I didn't have the Death Mask or forgot to equip it.
-worn Cerberus uniforms in ME2 or Alliance uniforms in ME3 (with the exception of the dress uniform for the end). For that matter I've always worn the leather jacket in ME3 and Kasumi's tux in ME2 (excluding my first playthrough where I didn't have Kasumi yet). Starting missions when you don't have a choice notwithstanding.
For squadmates:
-All ME2 squadmates got switched to loyalty outfits as soon as possible with the exception of Kasumi. For Miranda I've modded the files and replaced her white outfit with her loyalty one so I never see the white one past the starting mission.
-ME3 squadmates: Javik always gets switched to black, Liara always gets switched to armor, EDI always gets the black catsuit. Tali gets switched to her second suit. I used to switch Vega and Garrus but their defaults are growing on me.
I've never purchased the Collector Armor. I really hate the way it looks.
My Vanguard wears Hahne-Kedar Chest and Arms, Serrice Council Shoulders and Armax Arsenal Legs with the default N7 colours, no hemlets, they look too stupid for me. It's by far the best looking combination I could create which at the same time also benefited my Shepard, it's a mixture of Style+Substance. I refuse to wear any of the Rosenkov pieces, they are so damn ugly.
