Never imported a Mass Effect 2 character into Mass Effect 2
Considering the way NG+ works in ME2, that is probably advisable. I tried it once with a level 30 character and got my head stomped in. Without all your upgrades you are pretty weak at the beginning of the game.
If you don't take the paragon/renegade interrupt when talking with Vendetta, Shepard looks to his/her left with a worried look.

-not pushed the merc out the window in Thane's dossier
-not taken the renegade interrupt when it was the fastest way to get things done
-chosen Morinth over Samara
-tried convincing Garrus not to kill Sidonis
-betrayed Wrex in ME3 assuming he makes it that far
-cured the genophage with Wreav in charge
-sided with the geth over the quarians
-freed the Breeder
Never destroyed Collector Base and never will. Regardless of no-consequence in ME3 and Cerberus going bonanza with it, at the moment of choice, destroying it is just wasting potential priceless intel.
Destroying the base is one of the most idiotic paragon decisions in the series. I only destroyed it on pure paragons but I always had trouble clicking "No, we don't need it" after TIM makes such a compelling argument. Even though I was role-playing a short-sighted idealist it went against my gut so strongly.

I think the only thing I've genuinely never done is romance Ash, Tali, or Jacob to completion. I do have a bunch of things I only did once and vowed not to do again.
I've done everything. Optimal paragon, optimal renegade, sub-optimal paragon, sub-optimal renegade. I covered every base.
Actually, I never chose Refuse, because by then I'd concluded these stories and I only replayed the ending for two Shepards -- who go Destroy and Sync. And that option simply doesn't fit any character I've done, not even the lawful goody-two-shoes IMO.

It's been, ah, speculated that that was actually her intent with the "parting gift" scene, whether you romanced her or not. Was it just a benign memory transfer, or was she trying to "save a piece?"
Yeah... most of my Shepards turn the offer down.
I've never read the scene like that. Only seems to be those with 'special' issues with regarding old blue face.
