I had this problem the whole time that I played DA II, at the end I gave up. Looking around, it seems that the culprit is the combination of DX11 and NVIDIA cards, since the game was designed to run in DX9. My computer doesn't give me the option to roll back to DX9 and I think that it happens with most recent GPUs.

Like Bob Walker I think meant to say, reloading the last saved game (not the "file") solved the problem for me until I moved to a different area (even inside the same map)
Oh I get it. That would be indeed a monumental pain. I fear no one will find a fix for this, considering it's a driver issue.
Same issue here on a GTX 770. Maybe having the game as part of a Humble bundle will make a patch a priority, but I'm not hopeful.
This bug was fixed at some point but then re-introduced in the latest drivers. The best bet is to post in the Nvidia forums and get them to re-fix it in the drivers.
