Where can I find this PC mod that lets you change the death blow animations?

Well, according to the video that came with the Collector's Edition at least, deathblows trigger when you are "Dominating in combat". I find that they will usually start triggering when a melee character has killed quite a few foes in succession, or killed them very quickly.

Assuming the "Messy Kill" attribute on the Ageless wasn't bugged, that would be one way.

Studying the game as a role player, a huge contribution to Messy Kills would be done to bosses or powerful targets (you know, yellows and oranges are huge targets of messy kills, save few such as Uldred)

As far as the normal enemies go, I would dare say it is either entirely based on unbiased, unprejudiced chance, or it is based on melee critical hit chance. Perhaps the latter, but I doubt it, otherwise I would be spamming messy kills at end game point.
