My guess is that whatever broke him had to do with demons and resulted in the loss of his eye. Just a guess.
Maybe he had to kill someone he considered innocent or a good friend due to them being in contact with a mage that couldve "corrupted them"
This is actually what I think most likely, him becoming weary of killing people who only want to be left alone (granted some of them are actually rebels and violent, but I imagine some are just running away). Maybe he has to kill a mother with a little kid and it breaks him, or something along those lines. Given that 'he can kill almost anything he meets' I don't see him being broken by losing a fight.

It would be a very interesting story arc! And not one that has been featured on any Bioware game yet (pre-DA most companions with potential like this were just outright evil or mercenary - Canderous, is the one that comes to mind, and Sagacious Zu).

You have to talk to his back the whole game. He'll constantly be hammering out his sword/axe blades, or repairing armor, or painting daisies on an easel.
