I'd imagine it would be as simple as having the largest set of horns, say, the ones that look like the Arishok's, trigger a 'my horns are bigger than yours' dialogue.
Which I imagine Iron Bull would react to with bafflement, since qunari within the qun don't place any real value or pride on their horns.

His head seems kind of small to me, honestly. It's the first thing I noticed about him in the group shot.

I think it is just because his horns are so large.

I think it's still partly perspective since we're looking up at him, significantly more so than the other characters. But even with that in mind, his head is probably still going to be a bit small (just not as small as it appears, hopefully), because of his girth. Men that muscular and broad shouldered can appear to have small heads and nonexistent necks.
in all the art including the fan art it looked like his face was too small for his head, but then i looked harder and saw that his horns were so freakishly huge that it threw my perception off
