Sera fell after an attack. That either means she is killed, unconcious or magically paralyzed.

Personally I find that moment far too random for a character to die from it, it's completely outside the players control. I rather doubt she's dead then.

Maybe she was not hardened or the player did something to make her run off

I've been reading it in other threads too and the specific comment is she's "exclusive to a female Inquisitor."

Oh wow, I kinda figured this would be the case with Sera, but I also kinda feel sorry for the straight guys, she sounded like the sort of character gamer dudes tend to like (translation: she sounded a bit like Tali).
