It's also possible the artists used older characters as a 'base' for drawing the new ones, causing some similarities to arise on the go. I think IB looks remarkably close to the Arishok in terms of the overall character model.
There's also the general sense that the outfit is less something someone would wear and more a semi-random collection of visual elements the artist thinks are cool.
Actually, it's exatcly the concept behind Sera's outfit: she wears it because she likes it.
I always attributed Bethany's neckline to Varric's storytelling, myself.
