Ah, okay. Them saying that we know that got me nervous. But if it is like ME2 where they all can die but also can all live then I'll be fine with that.
I doubt Bioware would spoil ahead of time that companions will have forced deaths.
But yeah, its most likely gonna be optional deaths, if you make wrong decisions, etc.

I doubt any deaths will be mandatory as far as companions go, so an ME2-esque scenario seems likely (though not necessarily restricted to the final mission).
Not too long ago Mike Laidlaw said something about it being possible to finish the game with only a single surviving companion remaining, which might be interpreted as an indication that all companions can die over the course of the game at one point or another.
Either way, it's completely normal. Wouldn't be the first time for sure.

He specifically said that it would be very difficult, because you'd have to play a very bipolar personality, but you could ****** off all but one companion enough that they won't follow you. I don't think that had anything to do with character deaths.
Anyway, on top of Sera's "death" being optional in that scene, it's likely that the person flung through the door can be different depending on who you take with you. I doubt it's specific to her.

To be honest, after ME3, I've learned not to get my hopes up as long as character deaths are concerned. I still doubt that any possible companion deaths will be mandatory in this game, but I sure as hell won't be holding my breath this time around.

As I said, as long as no deaths are forced but instead if I play right they all get out alive, I don't care. But if we have another "One of these characters need to die" moments, Bioware will have lost some of my respect.

I read the 'no matter what you do some characters will die' thing somewhere too. But I can't hunt it down conveniently at this time so I'm not going to use it as solid evidence. I too hope there are some very logical steps leading up to it as well. (And that what happened to Iron Bull, Sera, and Leliana was just a worst case scenario of a sort.) Like if your PC totally (or for the most part) opposes the ideas and morals of a companion they may die for their cause. Or in the case of ME2 where the suicide mission could be a complete flop BUT you do have the possibility of saving everyone. Deaths that can be preventable if careful are what I prefer. Just not a SUPRISE!VIRMIRE incident. I wasn't entirely fond of that. Maybe for one or two companions, but I'd be sad if we lost around half of our companions/advisors due to surprise!Virmires throughout the game along with deaths we could avoid.
When they said that I think they meant it more in a "you can potentially end up with just one companion if you screw up" than in a "almost everyone will die horribly or leave without you ever getting to save them or keep them from leaving" way. That's how I read it, I might be wrong though.

To be fair, the first openly gay char in DA was that guy only male Hawke can seduce in Mark of the Assassin. Dorian and Sera are the first gay LIs though.
