Personally I think that the female courier from "Fallout: New Vegas" should qualify. I know Jim Sterling doesn't count user created characters, but considering that it's more or less impossible to create an attractive character, male or female, with Fallout 3/NV's character creator, it fits.

The point is that men can don't have to be attractive.
Women do have to be attractive.
A trend in and of itself can be sexist.
Those who are part of the problem are not always sexist, but the trend is.
Let's say there are for example a lack of non-whites in video games.
The developers who produce games where only non-whites appear are not necessarily racist.
However the trend of non-whites having little to no representation is racist.
Using the same logic:
You don't have to be sexist to contribute towards a sexist trend.
MEh. I don't care about trends. They are after all, just trends and changing.
Nitpick - I don't think a trend can be racist.
A trend isn't a living, rational being and cannot make any decision. Racism implies intent and actual feeling towards a group, so I don't think ti's a right word to use.

They don't need to be ugly, the problem isn't with pretty women, the problem is with not making them realistic. Realistic isn't a woman fighting on a battlefield whilst wearing make-up. Most of the women NPC's in these RPG's are often wearing make-up, even Aveline from DA2. Remove that, give them realistic armor, an interesting personality and you've got your problem solved. Although I admit, not every major character has to be good looking. This isn't solely a problem relating to female characters either.
Hopefully we start to see different body shapes too like with what Dragon's Dogma did. Not every man is a hulk and not every woman is a match stick.

Because sex appeal and lack of it are totally subjective to peoples' perspectives. Not everybody finds big-tittied, scanty-clad women slapped all over a game cover or a game attractive. Some people find it sexist and annoying, or downright ugly.

I like a happy-medium.
I don't want to play a p0rn game but if the main character has to be a female, I want her to be attractive and not ugly as I have to stare at them for the length of the game.

I don't either. But I try to make my character that I am going to be playing on for 100-300 hours nice looking. In fact I mostly play in 1st person but it makes me fell good when I know that my character looks as decent as I could make them look.

I have an idea.
You take a photo of yourself.
You then upload that photo onto your console.
Then on any game that has a face creator you can use the character creator to approximate your face.
Imagine a protagonist that looks like you!

Okay maybe I might have chosen the wrong word when I suggested that a trend could be sexist.
Obviously trends are not conscious beings.
That said, I think the trends in and of themselves are worth talking about.
My hope, is by highlighting gaps and examples of under representation this will pave the way for greater diversity.
This is why I am not attacking developers. I am attacking trends instead.
If you contribute towards such a trend don't worry. You're not under attack. Your fine. Keep doing what your doing.
If you have a particular vision go for it.
We have had a good discussion on this thread.
Every contributor has had something interesting and helpful to add.
I am including the people on the other side of the argument who feel this isn't an issue.
Both sides of the debate are welcome here.

A law that says black people have to sit at the back of the bus is racist
if laws can be racist then so can trends
there is nothing about the word racist or sexist that implies either have to be applied exclusively to sentient beings
