I have a quick question. Is she Lady Cassandra or Lady Pentaghast?
I know in the English nobility, they signify different things.
Not that it would matter very much to her, I'm sure, but...which is it?
Seeing that she's royalty (and a high-ranking member of the Chantry), I would say Lady Pentaghast. IIRC, not very many people (if any at all) actually call her Lady Cassandra. When they use her first name, they just use her first name.

Do you actually know this for a fact? AFAIK, there's a distinction. I know that if you're the daughter of say, an Earl, you're Lady Elaine.
If you're the wife of Baron Sutherland, OTOH, you're Elaine, Lady Sutherland. I'm pretty sure if they called Lady Sutherland, Lady Elaine, that would be a huge no-no.
You might not be interested and think it's beyond trivial, but I don't think a lady is a lady is a lady. There's distinctions, and the distinction relates to rank.
There are in fact no distinctions. It is the female equivalent of Lord, in such settings, and is meant as a sign of respect towards someone of higher or similar social stature. The problem with your example is that last names did not exist in the same connotation during the use of such titles. At least not in the same way as we use them. Her name would be Lady Elaine of Lancaster, or Elaine de Lancaster, or Elaine Lancaster. Last names are a more relatively recent thing. Thedas isn't really a good medieval analogue.
In the UK, the general rule about this (at least according to wikipedia) that Lady "Name" is for those who the daughters of nobles, while Lady "Surname" is when they've gotten married and taken the husband's name?
Of course, Thedas doesn't seem to follow these rules as Josephine is referred to at various times as Lady Montilyet and Lady Josephine?
Best I can figure, people in Thedas use surnames for those of a higher social class than themselves (or if they're being polite), while those of the same higher class are allowed to use the given name, at least if they have sufficient familiarity with the person they're speaking too?
In Origins for example, Duncan respectfully refers to the Human Noble's father as "Teyrn Cousland", whereas most of the nobles refer to him simply as Bryce? We see numerous times that the lower classes tend to refer to individuals such as Loghain and Eamon by their titles of Teyrn and Arl respectively, whereas their nobles peers tend not to?
The only example I can think that bucks this trend is probably Arl Howe, since no-one refers to him as "Rendon", but I figure that's because he's so unpopular that barely anyone has that level of familiarity with him? Or maybe he just hates his given name, since most of the people who actually consider him to be a friend (like Bryce Cousland) don't seem to use it either?
Cassandra probably would prefer just to be addressed as "Seeker Pentaghast" or "Seeker Cassandra", since it's a title she earned for herself? From Josephine's comments throughout the game whenever she mentions trying to leverage Cass' family's connections for support, her noble background is something of a sore spot with her and she doesn't like to be reminded of it?
It's probably why Cassandra comes to respect Hawke as a self-made individual, as we learn in MotA from Sebastian that Hawke turned down the title of Lord/Lady Amell for that reason? Refusing to ride the coat tails of their noble heritage in favour of hard work, is probably what resonated with her?

Mayweather should have fought Cassandra instead of Pacquiao. Cassandra is a woman, so he probably would have actually tried to hit her instead of nonstop hugging and guarding like he did to Pac.
