I think her scars are just there do to the fact that they can put them their now. Better character animations and all. I mean if you want to really get into it it looks like she has aged 10 years since DA2. But that again is just do to how limited the DA2 character animations were.

Given that Nevarra is somewhat inspired by Prussia maybe the mark on Cassandra's cheek is a dueling scar?
