Don't panic! The death card in Tarot symbolizes change, and Cassandra is going through some in the game. Her world view is changing from black and white to shades of gray.

Well, the Collectors edition of the game does come with a pack of tarot cards, so it's a bit difficult to avoid the connection...

Matt Rhodes confirms that the UI cards are what are used in for the tarot cards (not just the party selection). Which means we might have multiple variant of the same character and some stuff might not even match tarot that much (like Varric's chest).

The Death card shows Death himself riding on a white horse, holding a black and white flag. Death is portrayed as a skeleton as the skeletal bones are the part of the body that survives death. The armour he is wearing indicates that he is invincible and unconquerable. Indeed, no-one has yet triumphed over death. The horse that Death rides is white, the colour of purity. Death is therefore the ultimate purifier. All things are reborn fresh, new and pure.
