Lulz. A daily occurrence for me.
Personally I start wave 1 by going down almost immediately, and applying some Gel. I make sure to go down a few more times throughout the match and mash that omnibutton, though by that time my partners are usually quick to pull me back up and continue the action.
I play careful depending on the character I use, a mix of offensive and defensive. My solos are not quick ones, lets just put it at that.

I generally go all out all the time. I'm not good enough to be able to afford to mess around, especially as most of the PUGS I meet tend to suck.
As for your points:
1 - 3 - I agree completely.
4 - No. There are only two times that I will use missiles. To save the match, or during the wave 10 objective. I'd much rather save all my missiles for wave 10 if possible so that they are available to ensure the objective get completed.
My style of play changes based on what tools I have available, and that changes from kit to kit.

To those who didn't, it's probably because your style leaves you on the bottom constantly and you end up laying there helplessly wave after wave. I'd be pretty frustrated in that case too, but you have all the tools you need to turn things in your favor. I personally think being on top all the time is overrated too; it's a great feeling to expend yourself for the entire battle and finish in close proximity to your equal. In that case, it doesn't matter who finishes first or 2nd, as long as you and your team extract successfully and you are pleased with your performance.
It's fantastic.
And involves a lot of grenades.
So whatever that is considered.

Specifically regarding missiles, if it's wave 8 or 9 in a game which didn't have targets on an objective wave, and I'm confronted by a spawn, I will use a missile 99% of the time. I have 6 of them, and know I won't need more than 4 in wave 10, so what else are they for? Certainly not for use on wave 11!
Am I the only one that loves when people nuke spawns on 11? Could be that I am almost never level 20, which means if they want to spend their missiles to level me up, go right ahead!
My play style changes pretty drastically depending on the character I'm playing. With the Fury I feel like it is my job to kill everything. AM III and bring the biotic mosh pit to whoever I can find. BE till its dead. depending on the map (glacier) I may leave a trail of mostly dead broken bodies in my wake for others to clean up, but otherwise if I see it, I kill. Other end of the spectrum with the Volus engineer I usually try to stay with people, drop the RM on choke points, spam PM, and generally try to keep the people I'm playing with alive.
I also find revive style kind of interesting. I'll try to revive someone unless it is completely impossible, or it works against the objective. You die on the far side of the map while we are hacking/I have the pizza/currently hacking a device, I'm probably not getting you. Otherwise I try to revive people. If they are good, I want them upright shooting things. If they aren't good, I've been there, and generally it's still nice to have the enemy shooting at other people. Usually I end the game with a revive trophy, or the group I was in was so good/rage gelled so much that I couldn't find 5 people to revive.
On the flip side of that, nothing makes me angrier than when someone could have very easily revived me, but couldn't be bothered to stop shooting to pump up their score.
