I have been all over. Found 1 - in viscount's keep, 2 - recently opened passage in sundermount, 3 - in the chantry. Anyone help with location of the others? Thanks.

Well there's one in the bone pit and another in a cave you find if you have labels on in options up the wounded coast.  You probably passed by the cave a  bunch of times and didn't see it. 

 

I think you have to let some blood mage live also the one at the blooming rose who tries to control you.

You don't have to let Idunna live for this quest. If you kill her, you won't get the letter, but the quest will activate when you find the first tome.

Btw,you can't finish the quest if you read/take any of the Evil Tomes,you must destroy them,but you can read/take the last one in Darktown. (The Forgotten Lair)
