...but I didn't get the "perfect ending". 4200 EMS which should be more than enough according to all the guides. I got the extended cut, talked the Illusive Man into killing himself etc. so I have done everything right? Any ideas on what I need to do to get that ending? I really don't wanna keep trying things and going through Earth again and again.
So I played through ME3 again and got to the end...

Are you sure your ems was 4200 and not your TMS(total military strength)? If your GR(galactic readiness) is at 50%, then your ems(effective military strength) is only 2100.

No worries now. I got to 4000 EMS playing online after the Cerberus base. But now I went back to before the attack on the Cerberus base and I got the good ending. I think EMS is locked once you start the attack on the base.

You're right. That's when the EMS check happens, probably because once you're attacking the base all decisions that can change EMS have already been made.

My ems was below 3100 when I started the Chronos mission and when I finished it was slightly over 3100(heart) which eventually gave me the breath scene. So I guess they take into consideration that you either get the heart or brain, depending on your decision from ME2, before attacking Chronos.
