While they've said they're not going to pander to fans, a wise businessman balances between his vision and his fans' wishes. It wouldn't surprise me if they saw how popular he was and wrote a role for him. DAI looks like a good one for having lots of mages and templars, why bother writing someone new?
Plus he's never been a companion - just an NPC.
That's why I chuckle when some posters complain that there is "tons of new characters" or "it's only old characters" returning. For a lot of folks, these former NPCs are almost blank slates. If Cullen is a companion, then this is someone that most players have only talked to briefly. Outside of those that are so inclined as to start scouring for every bit of Cullen's potential dialogue, and every bit of codex he's in, we really know little about him. Cassandra is my other example, since not everyone saw the anime and knows her story.
Other than Varric, it looks like none of the followers from the previous games are going to be in my party. That's more than new enough for me. ![]()

In Dragon Age Origins, we temporarily partied up with Merrill. We partied up with her permanently in DA2. We partied up with Anders in Awakening and did so right off the bat (and most people have played Awakening) and then we got him permanently in DA2. Isabela was never a temporary party member (unless orgies count as parties) in DAO - just an NPC. And we partied up with her permanently in DA2 as well. Also, in reverse, we partied up permanently with Zevran in DAO and then did a quest with him (him in NPC-form) in DA2. So it's not completely unheard of that Bioware makes former NPC's into companions and former companions into NPC's. I'm not exactly sure why so many haven't recognized this pattern...
Edit: I almost forgot. The same goes for Nathaniel Howe and Alistair. Nathaniel was an Awakening party member and an NPC in DA2. Alistair was a party member in DAO and a (very brief) NPC in DA2, helping us fight off the Qunari.
