Does this problem persist if you reload/restart the mission? Also does it REALLY only give 1xp or that all it SAYS it gives? When you look at your Shepard's level, is it really only increased by one?

Nah, they wander around with their brains out in the open. Marauder plates are blown off clearlyI always thought that was just the armor they get from marauder shields being blown off.
In the elevator shaft during the Citadel coup, Shep is bareheaded while the squadmates have their helmets on. (Possibly that's true in every mission with breathable air?)
If a femshep is pursuing a romance with Kaidan, you get an obvious closeup of Kaidan's ass when you re-recruit him just after the coup. (I don't recall any similar shot when broshep re-recruits a romanced Ash.)

Yes. Its random. I've tried many different ways to see what causes it and found no reason why it happens.

I never bother to reload or restart. Most of my games are imports, so I'm already at a decent level.
But I have no idea why it happens. Maybe one of the PeaSee Mustard Race could sort it out,

If you stand directly in Anderson's path, he'll push you forward. You don't even have to run.

Yeah, you need to chase him during other sequences. This one I got ME3 minimized and then restored to see the cutscene have passed
So apparently, if you spend talent points then choose Undo Upgrade, it purges any "tech armor" you have activated (Tech Armor, Defense Matrix, Barrier, or Fortification).
Not particularly fun if you are carrying -200% weapon weight. Luckily I was right next to workbench to lighten my load so I could stop the cooldown and re-apply my Tech Armor and Fortification.

There's a lot of panting sound files within the post Kai Leng conflict on Thessia for every squad mate(EDI too, surprisingly)and Shepard(M/F). Some of them I've never heard of before because there weren't that much panting. Some sound almost as if they're very hurt. This could prove that the original Thessia script was somewhat done but not close to being half done. I guess these sound files indicate the impaled Shepard and the choice between choosing which squadmate lives and dies.
So I found out that Shepard's pre-spar dialogue with James changes if you wait to talk to him until after Manae instead of before.
It changes even further the more you avoid talking to him as the game progresses, implying that Shepard doesn't care for him (
