Yeah I was disappointed that they just reused the same Alliance soldier models for all the bodies. It made the place look more like a simple military installation, since not a single civilian casualty could be found.

In ME2, complete MIranda's and Jack's loyalty missions. Side with Jack causing Shepard to lose Miranda's loyalty. IN ME3, do everything that needs to be done to keep her alive and she will survive
Instead of talking to Anderson after Thessia, talk to him after Sanctuary. He will mention the catalyst twice. The last line he says before he hangs up is 'Find that catalyst'. I forget what the other line was with him saying catalyst
When talking to the VI at Exogeni it will say Elizabeth Baynham. The dialogue wheel will show Lizbeth.
If the player gets out of the Mako before the cutscene showing Lizbeth running from the Mako, it will still have that cutscene if the player moves far enough forward to trigger the cutscene
After seeing Kaidan/Ash in the hospital when s/he regains consciousness, you can prompt him/her again for another set of dialogue. I had no idea there was additional dialogue since it places you by the door once the first conversation ends.
Yeah, this can be quite a surprise. The same thing goes for a talk you can have with tali as soon as she comes aboard. Well, assuming of course that she survived ME2. The game places you in control of the normandy in the galaxy map. You can leave it, return to the combat room, talk to tali and even invite her to your quarters prior to meeting legion or the geth vi. You can even ask her if she heard any news from legion. Again, assuming he was activated and survived ME2.
When scanning for resources on the planet Klendagon, I wasn't able to see the Great Rift. I looked on the wikia and found this explaining why.
When the player decides to scan the planet in Mass Effect 2, a default planet texture is used so the Great Rift Valley cannot be seen.
There was going to be a mission featuring Xen conducting experiments on the geth. This scene with the admiral is probably the result of delaying this mission for too long (like the bomb on the krogan home-planet or grissom academy after the coup).
I can see why it was scrapped. We had too many mad scientists trying to conduct brutal experiments already in this franchise. Not to mention that based on the evidence it may be that it could only be triggered by having both the quarians and the geth alive after their resolution. But I think they could make Xen one of the few survivors trying to act in a desperate manner, Having an entire mission being conditional is something that never happened to my recollection. Even when the mission was designed around a particular character or event there would always be parallel versions of it in case that particular character died sometimes featuring another character as a double.
I was watching footage of the final mission and realized that apparently the 2 brutes that destroy the wall hulk-style right before the missile battery section seem to have come out of nowhere. There was no door from which they might have come from, and I think they could not have arrived via air-drop. So I guess they just materialized there. I'm telling this based on a video. I could not see if they could have come from above. Or so it seems that banshees are not the only reaper enemies capable of teleportation..
When does this dialogue happen or was it scrapped and is just lying around somewhere?
Dantriges: There are lots of left-over files inside the game. Sometimes its even possible to reconstruct entire scenes from them. You can find several cases of deleted material on youtube or pieces of it in the mass effect wikia.
Just from memory, I do recall:
A different way to recruit the cerberus engineer on the citadel (in case only he survived) or the couple, in case both of them survived. Theres a mod to the pc version that will restore this scene on the purgatory nightclub.
Zaeds conversation about the blue suns, cerberus and what he was doing recently.
The files about Aria reporting victory over cerberus at omega, prior to the omega DLC being released.
Characters like Jack, Jacob and Zaed attacking during the last mission on Earth. I have the feeling that it was something similar like assigning a team leader for the rest of the squad during the vent and corridor sections of the collector base, when sometimes they would report their progresses over the comm.

From a look at these files, one can see that as good as ME3 is, it could have been even better, The final mission in particular was supposed to be a lot more complex and play like the collector base from ME2.
For the first time I had a playthrough where Tali (romanced) didn't flirt with my Shepard on the geth ship, nor did she make a joke when saving my Shepard from falling from the platform. I was actually worried that I had messed up somewhere and failed to continue the romance earlier but that turns out to not have been the case. I'm not 100% certain why it turned out this way. My character was WAY more renegade than usual (cheats) and I stayed "professional" when talking to her in the war room (but did confirm the continued relationship with her in my cabin). Maybe the way you behave towards her determines whether or not she is "playful" on that mission.
This will probably be something everyone else noticed and loved before, but I think this is the first I encountered the conversation between Javik and Vega where Javik makes up a fake joke to make fun of Vega. I lol'd pretty hard - Javik is a total hardass and jerk, but my renegade Shep loves him for it.
After sending Vega back to the shuttle and dealing with the Cerberus guys, a Cerberus APC can be seen. I'd be curious how it got there since it appears to be too tall to get in through the entrance Shepard uses to enter the building
If Ashley is in the game, she will say 'What's that smell?'. Before that line, there are containers that can be seen on the left, when looking through the windows, with C7 marked on the them

Replaying the game twice now after 3 years, I see a lot of stuff constantly reused. Animations, models, backgrounds, etc. Heh I made a joke to myself that Mass Effect 3 is looking like some sort of high budget action movie, that cut corners at every possible chance to focus on big action stuff.
If Garrus is taken to Eden Prime, he will somehow remember Ilos even though he was never recruited in ME1
