When you enter the building on Sanctuary, the window on the left has a spot that can be activated with the squadmates having a couple lines of dialogue.
After healing Wilson in ME2, he says to use overload to explode the cannisters. The cannisters explode before he uses overload and a moment later you see him use the power.

I don't know how but I always missed those last video logs of the Illusive Man in Priority: Cerberus Headquarters, the ones about his upgrades. I've just walked right past them on the way to the final fight with Kai Leng.
When you take the tram from Hermes Station to the Antenna Dish, Dr. Archer will notify you that the VI is realigning the Antenna to a new target. If you look at the Antenna you can see it moving.
On the Shadow Broker ship there are two monitor panels that are laid out in a three by three grid of smaller monitors. These are the same monitors you see when you meet Veetor on Freedom's Progress and show the same footage that Veetor shows you.
If you have Liara visit the Normandy after the kidnapping, but before going through the Omega 4 relay, she does not mention meeting Dr. Chakwas. Instead, she says she would be worried if Joker didn't say whatever he said, a mention of Vaenia if a romanced Shepard, embracing eternity if non-romanced. I imagine if Chakwas dies the dialogue is the same.
At the beginning of Chronos, if you don't find a console in time, a critical mission failure screen appears.
If your ems is at 2900, you can still get the breath scene. I just finished another playthrough with my ems at 2907 and got the breath scene by "saving" Anderson. Apparently he's worth 200 war assets if "saved". And as I posted in this thread awhile ago, if you let TIM kill Anderson you lose 100 war assets.

Well, about two dozen playthroughs and I never noticed that. I will have to look for that the next time I am on that ship.
Some things I have noticed before but others may not have:
In the Leviathan DLC underwater in the diving mech, you will notice that on the right side of the mech's display are the lines of code from ME2's hacking minigame.
During your mission at the Silver Coast Casino (Citadel DLC), you can hack two ATM machines for about 3000 credits each... and some Renegade points.
If your Citadel party is a quiet one with only ME1 crew invited (aside from the mandatory invites), at the beginning Wrex and Kaidan will reminisce about Ashley. Presumably Wrex and Ash will do the same for Kaidan, but I haven't tried that yet.
After you've talked with the PTSD asari at Huerta, there are a couple of nurses in the Presidium Commons talking about the case. One of them says that she's making the asari freak out because she looks like that farmgirl, so I guess we know what

How do you get Udina to change his mind and go for a human lead Council? AFAIK paragon/renegade does not influence it.

I believe for a Paragon ending, Udina mentions a human led Council, and for a Renegade ending, Udina mentions an all human Council.
Yeah I had paragon ending. Nearly 80% full renegade bar, but still ![]()
Also picked 'concentrate on Sovereign' instead of 'let the council die', dunno if that changes anything.

As the Crucible fires the destroy wave and explodes, a couple of dead reapers can be seen
When gaining control of Shepard, after the FOB, if your fast enough, Shepard can run in front of the vehicle.
If edi is with you when defending the missles, stand beside edi when it has dialogue and see the mouth isn't moving, but when the first set of missles is fired, you see the mouth move
When the first set of missles is fired, the squad remains in the same spot, but when the second set of missles is fired, all three move away from the spot they were at.
The music that is heard during the ex-Cerberus scientists mission is the same music heard in the Overlord dlc
The ex-Cerberus scientist mission has a few tracks from ME2. Like when first encountering Jacob outside of the facility, the sound that plays is the same one used when Miranda was questioning Shepard on the shuttle, and inside, the theme used is the same one played during the Lazarus station escape sequence. ME3 does have a few others throughout game, like on Cronos, in the chamber with the remains of the human reaper, the Infiltration track that plays during the first part of the suicide mission plays again here when the first phantom makes the scene.
When doing Kasumi's loyalty mission, someone will mention that Vido Santiago was attacked by Zaeed Massini and thankfully was able to escape. Of course to get this dialogue, Zaeed's mission has to be done first and the workers are saved.
In Hock's vault, he has 6 statues of Krogan. A statue of an Ogre from Dragon Age. Two miniature replicas of a mass relay. A statue of the Inusannon, the species before the Protheans. The statue can also be seen on Ilos in ME1.

ME3: While sneaking onboard the Normandy in Citadel DLC, and you're supposed to be quiet: Fire your gun, and the mercs will know you're there and open the hatch and attack you. Can be a pain since you can't really move anywhere, and don't have any cover either.
ME2: I like to mess with my squadmates. After Tali's loyalty I went to talk to her, and then didn't pick all the dialogue options. Went back later to talk to her again, and Tali mentions something similar to: "Trying to talk to my ships bosh'tet captain." Guess I wasn't a very good listener.
