ME3:
When first landing on Sur'kesh the salarian who reports your unauthorized landing will have no moving mouth whenever I play as femshep, but the mouth animation always work with my malesheps.
At the start of ME3 when you're running around with Anderson, once you get to the part where the Alliance ship goes KABOOM; as you're falling down you may notice that down (at the very left side) below is another copy of mr. Anderson himself just standing there. Best way to see it is with a flycam.
A bit of skipping on Menae:
After repairing the communication tower the turian will tell you that he'll contact you as soon as he gets any new information about the primarch. To which Shep will respond: "I'll help your men while I wait." ... more or less.
But after Shep has finished that line, you don't actually have to wait. Just run past all the husk, the gate will open and let you in, and then the turian guy will call you and ask that you return to him, by which point my Shep is usually standing right next to him.
"Damn, Shepard, you're fast." ... too bad I have to headcanon that last line ![]()

Some days ago I was watching someone play ME1 on the PS3 on twitch.tv when I noticed something new, to me at least.
He was giving his squad orders, attack that target, follow me..etc.. all that fun button clicking. Which isn't new, but on the PS3 version those commands have ingame dialogue you can hear Shepard giving the spoken order. I don't know if the Xbox has them, but my pc has always been without voices for the squad commands.
Miranda's such a... ugh.. on the phone during the suicide mission.
If you romance Tali all the way from the start, she will give you a photo of herself with no armour after resolving the Geth/Quarian conflict. Also, before the party in ME3: Citadel, go to the casino and look for a suspicious device by the side of one of the machines if you have Kasumi already. She will be attempting to rob the casino with some aliens. You can decide what to do about it.

ME2:
Take Morinth and Grunt to Tuchanka, go to the hospital area, and click on Morinth's dialogue there.
Morinth will say a pretty interesting line, to which Grunt will respond something akin to: "Shut up, asari ****."
This is really interesting. If I remember correctly Samara has a completely different dialogue on Tuchanka. I wonder if Grunt can actually tell that Samara isn't Samara. At least the krogan shaman uses smell to do a quick check on Grunt, during Grunt's LM. Might be that Grunt knows about the change, but doesn't really care, so he never talks about it like Kasumi, or Kelly.
If you don't talk to Lieutenant Talon, apparently no Salarian shows up to warn you about the engineers setting up turrets. Instead, you cut directly to the part where the first turret is set up. After the battle on that level is done, if you go back to where Kirrahe would have been, you will find a dead Salarian. Javik comments "The Salarian is dead."
A couple more things I just found on Surkesh. If you move too close to the exit from the level where you have to reactivate the power supply, Wiks (and I suppose Mordin) will tell you you need to restart the elevator before you leave. Also, if you kill the Atlas that drops in fast enough, no additional troopers will drop in.

I've talked with Talon several times and never had him warn me about turrets being set up. The only Salarian that has showed up is Kirrahe whether you talk with him or not. Of course Kirrahe has to be alive for him to be there.

I am probably late to the game here, but I finally figured out why you get some of these seemingly pointless hack/bypass mini-games in ME2; seemingly pointless in that if you fail the first time, you just have to do it again (and again) until you succeed. It is to keep the player from triggering the next cut-scene/confrontation before the current battle ends.
I was watching ChristopherOdd playing the first Leviathan mission and was wondering how he would deal with his first encounter with Banshees. But he ended up missing those two Banshees that drop in at the end of the first battle because he hit the door lock that triggers the next phase of the fight before they could attack him. That's when it occurred to me that if not for the mini-games in ME2 you could simply run past all the enemies and trigger one cut-scene after another without ever having to fight.
(I know, quoting myself. But I wanted to give this post some context)
I found one way they managed to avoid the "cut scene bypass" phenomenon in ME3. I just did Tuchanka: Cerberus Attack on casual mode and since this is the equivalent of "Shepard can't die, ever" mode I decided to just run towards the switches without bothering to fight. Well, those switches are on a timer and if you get hit by enemy fire while the timer is counting down, then it stops and you have to restart. This seems to me to be a better way to avoid having the player get to the next cut scene without fighting than by using a mini-game. OTOH, for the final switch in the sequence, I was able activate the cannons and get Steve's dialogue while there were still enemies about. In fact, we cut to the loading screen before the last enemy was dead.
In ME3 on the N7:Cerberus Abductions mission (Benning), if you scope the dog tags, you can see a picture of the Normandy on it.
If you wait to go in Bailey's office before Chronos, he and Khalisah will have the same conversation as when you first see them near the beginning of the game. When talking with her before using the elevator, the dialogue and interrupts are the same.
Normally, if you go too fast through a level you miss out on a lot of dialogue. I found out today that sometimes you can get dialogue you haven't heard before if you move at lightning speed. I was racing through the underground portion of Tuchanka just now and Shepard kept saying "repeat that, I didn't catch that." as I sped through the underground as fast as possible. I also found out you can't jump over that one bridge until Kalross goes by. The path forward is blocked until Kalross makes its appearance and passed by.
You can get cut off by the hanger barrier when rescuing the Scientists and not get back over it.

Something similiar like that happened to me. I had Ashley and Garrus with me. When the door opened, I was slow to move forward causing the door to close blocking my way forward. I couldn't do nothing and ended up reloading the game

Any idea what triggered to have Liara shoot Kaidan instead of the other squadmate? I know I've had a few times when Ashley/Kaidan were shot, but not by Liara or Shepard.

I am guessing it depends on the order selected. I had Liara and Javik in the squad and I picked Liara first.

Whoops, looks like I was wrong. The cutscene looked like Liara pulled the trigger. But I just talked to Javik and he claimed responsibility.
Didn't realise that if you kept drinking shots at the citadel in ME3 that you wake up next to Aria on the couch and if you do it again you wake up in an elevator outside purgatory. I seriously just went there and had 1 shot and was disappointed nothing happened so I never tried it again, wasn't till my 3rd play through I tried having more shots.
During the confrontation with balak in d24, shep turns around to confront him to talk him down - when he agrees to join forces she turns around again as he walks away.
Yeah, this always bugged me a little, and it only works when you use the paragon dialogue, since Shepard doesn't turn to face Balak if you choose the blue option.
When talking with Adams about the GX12 thermal pipe, Ken will mention that a crew member was killed, if the shield upgrade wasn't purchased, but if you do get the upgrade after the suicide mission, Ken will not mention that a crew member died
