Screw the dog tags. Go to Benning to kill Cerberus, not to help out some silly ambassador.

Actually Liara & Miranda met during the course of the Redemption comics. Miranda recruits Liara to recover Shepard's corpse for the Lazarus Project. Or rather she makes the initial proposal of cooperation, and then brings Liara to TIM for the sales pitch.

I've read through this whole topic. There's a bunch of things I didn't see mentioned:
ME2:
If you have party members who were present on Eden Prime in some capacity in ME1 (e.g. Jacob) and visit the Derelict Reaper while they're in your party, they'll have additional dialogue mentioning their experiences in the first game.
This one was mentioned earlier, but I wanted to post it again: If you take Morinth and Grunt to Tuchanka (during his loyalty mission, if I'm not mistaken), they have a unique conversation where she speaks in her "non-Samara" voice for the only time in the game to another squadmate, and Grunt responds by calling her an "Asari s***", implying that he knows she's not what she seems.
Taking Legion along during Tali's recruitment is amazing - when you get to Kal'Reegar, he notices and calls Shepard out on having a Geth standing right behind them, and Legion comments that he is "working against the Creators". Also, when Tali is rescued, she will shoot at Legion unless a Paragon interrupt is taken.
Both Miranda and Tali have unique responses during Mordin's loyalty mission, where you find the injured Krogan. When Shepard is talking to the wounded soldier, he can either insult the Quarians (leading Tali to say that she's standing right there and can hear him) or Cerberus (causing Miranda to say that she's going to mention that in her report).
In both Lair of the Shadow Broker and Arrival, the villain has alternate dialogue based on whether you do it before or after the Suicide Mission. In addition, the Shadow Broker has alternate dialogue based on whether or not the squadmate you bring along has finished their loyalty mission or not.
ME3:
The Virmire Survivor has dialogue for both the From Ashes DLC (which seems to have been made for the purpose of doing it as early as possible), all the N7 missions and the Tuchanka bomb mission, despite the fact that they're only reintroduced back into the player's party two-thirds of the way throug hthe game.
Jack has additional dialogue for Garrus (and only him) if the player is using a female Shepard and is in a relationship with the latter.
If you never bother to talk with James (i.e. visit him on the Normandy or the Citadel), he becomes very angry with you if you attempt to speak with him in the London FOB.
DUring the battle in "no man's land" to fire the Thanix missiles at the Reaper guarding the Conduit, a gaffe occurs: EDI will appear on Shepard's wrist communicator to tell him/her about an update, even if EDI is already in their party.
Not necessarily something that's missed, but an odd little thing - when Shepard and Anderson use the Conduit to get up to the Citadel, the subtitles go back to referring to Anderson with the Admiral designation.
(Omega DLC): Harrot has alternate dialogue in Aria's Bunker based on whether or not you obtained a discount from him in ME2.
(Citadel DLC): If you brought Garrus on Mordin's loyalty mission in ME2, the former talked about how his favorite places to battle through. If you bring Garrus along while retaking the Normandy, he will make a reference to his dialogue during said ME2 mission while you fight in the CIC.

I never had that problem. For me there was no difference. I even created a thread about what happens when you don't talk with any of the characters
In a trilogy playthrough at the moment. I was visiting Flux when my cursor went upwards. I then noticed that the ceiling had a misty fog with some twinkling lights (like stars). I had never noticed that.

Really? This was mentioned to me by someone else, but I never tried it myself because I'm a completionist. Good to know, at least.
Omega DLC:
Talking with the batarian mechanic who wants some inverters. If you walk away before he finishes talking he'll react and say: "Hey, where are you going?"
Shepard replies: "Hold that thought." --or something along those lines.
Click on the batarian again to continue the talk.
Edit: Forgot about the woman who wants you to hack the terminals also does a similar thing if you walk away while she's talking.
I love that time when even Hackett can have some doubts about my Shepard:
"Shepard, you're the meanest S.O.B in the galaxy, but you're the only one who knows how to kill reapers!"
When I heard this it really surprised me because Hackett actually sounds upset when he says it, and I never expected to hear Hackett say anything like that. He's always such a cool, and calm character no matter what crazy stunts I do in any of the games.
This was during a complete messed up renegade playthrough. I think this dialogue came at some point after I let the reaperfied rachni live, and things went all bad over at the Crucible project. Been a while since I did this, can't remember exactly, but it's one of those times when you talk on the QEC with Hackett.
This dialogue can be heard between two Salarians on Sur'Kesh
Salarian soldier: Agree with them or not, our orders are clear: ensure the smooth handover of the Krogan detainees.
Salarian soldier: Who gave the order?
Salarian soldier: Our superiors. That's all that matters.

I found something I knew, but had forgotten for ages.
At the Normandy dock if you go to that little 'balcony' thing where the keeper is and move the camera around over the ship you get minor codex and XP points for 'thrusters' and 'hull'.
It's only a little but every bit of XP helps when you're leveling up.

Jacob wasn't in the comic at all IIRC.
On Palaven's moon, Menae, if you have Liara fix the comm tower and run back towards the area that Corinthus is located you get some interesting dialogue
James: Shepard where are you going? We just can't leave Liara up there.
A moment later a Communication Tower Destroyed screen pops up.

Considering how he tries to "fix" the computer console on Rannoch (the Geth Jamming Towers mission), that is not surprising.
After I did the Turian Platoon Mission, I decided to do the Cerberus Labs, Cerberus Attack and the Cerberus Abductions missions. When I went to Shepard's private terminal I got this message from Wreav.

they hit Tuchanka. massive bomb meant to cripple us. when we find out who is responsible, EVERY KROGAN will cry out for blood.

I went and talked with Wreav, Victus and they didn't mention the bomb exploding.
I went to the galaxy map and noticed the bomb mission was still on the map. I'm wondering why I got the message from Wreav about the bomb going boom if the mission is still on the map?
I ignored it and went to Eden Prime to get Javik. I got back and went to talk with Victus, Wreav and still no mentioning of the bomb exploding.
So I made a save at this point. I went to the galaxy map and noticed the bomb mission was still on the map. I went and did the mission and got back to hear Wreav and Victus argue.
So I went back to the save I made and instead doing of the bomb mission, I did the cure/sabotage the genophage mission. After the mission, the war assets indicated the bomb did go boom and it was not on the map anymore.
This part is my opinion. I believe that you can do as many misssions that you want, including Omega dlc and Leviathan dlc, after the Turian Platoon mission without the bomb going boom until you decide to cure/sabotage the genophage mission.
I don't know if this is a little thing and it's not something I just noticed. But I get the feeling that the lighting down in the ME2 lower engineering deck is lit specifically for the Jack conversations. When she talks about her past you always have the red lighting in the background and as you move past the facade you get more shots with the blue lit parts in the scene.
