The muzzle break on the level IV looks better than the level V. imo

Across from the observation room that Shepard passes, on his/her left, before turning left down the hallway to where Kenson is being held, is a cell and on the left wall is a painting/drawing of a reaper.

^ Just curious, did you get any extra assets from playing MP?.
My last playthrough I was shocked I had 11000 and change just from MP. I noticed it's very buggy though. If you reload or go offline they disappear.

I noticed some small, but somewhat disturbing during the ending. When you leave the dark hallway and go across the bridge, if you look behind you and to the right, you see a waterfall of blood, several in fact, pouring into a river of blood. I had to stop and just gawk at that for a while.

I just noticed that the collector pods on horizon have people inside them writhing about....

I'm on Horizon right now in my current game. I'll be on the lookout for this.

This chick, she'd dig that:

Erzsébet Bathory. Bathed in tubs filled with (virgin) blood to stay young forever.
You may think and wish I'm joking... but I'm not.

The Reapers must have beamed up an obscene amount of bodies if there's rivers of blood everywhere.

That was not the Reapers, that was the work of Kharn the Betrayer.

Okay, this is something I discovered second-hand, while (slowly and belatedly) watching through cap and gown's Adept playthrough in ME2.
So in Mordin's recruitment mission, after dealing with Mordin's stupid assistant Daniel, you get to that part where you are on the balcony with Blood Pack Boom Squad Vorcha firing at you from another balcony and other Vorcha down below.
Now, I usually go down the stairs at that point and kill all the Vorcha and the few Krogan that appear.
Now apparently after watching cap and gown's playthrough of the mission, if you stay up top like a coward play defensively by staying up top on the balcony, no Krogan or Vorcha Pyros spawn down below.
(Note: I am merely joking with you cap and gown. Please do not take my lame attempt at humor seriously.)

After climbing the ladder while chasing eva, a cutscene happens and the player is awarded either 1 experience point or 150 experience points. I have no idea what causes the difference in the points received

I wonder if it has something to do with the average distance you can maintain when chasing her down.

It might.
In my current playthrough, I chased her and did get 150 points. At one point I did pass her and she sprinted passed me.

I've had random moments where instead of getting 1200 credits from a source I get 1 or 2. I think it's a bug?

I want to add to the link above
If you "save" Anderson, the player receives 200 war assets. It doesn't matter if your Galactic Readiness is 50% or 100%, the player receives the full amount. I wonder why that is when all the other assets the player only receives half if his/her Galactic Readiness is at 50%

By that point the end game has already started and EMS probably converted to a true value. Everything added afterwards gets the full value because there's no leveling anymore.
Or so I presume. The point where this conversion happens is most likely, if it happens, after Cronos.
Fairly certain it does btw, as your EMS determines how wel Hammer and Sword do.

The problem is after leaving Chronos, the player believes he/she has enough for this ending or that ending and ends up not getting it. That player will be scratching his/her head wondering what happened
example
The player heads to Earth with 3175 ems believing he/she will get the breath scene, but because the player lets TIM kill Anderson, the player loses 100 war assets therefore not getting the breath scene. The ems is really 3075
edit another example
The player heads to Earth with 1650 hoping to get the vaporize ending and "saves" Anderson, he/she will not get that ending because the assets is now at 1850.

Not saying it was a good thing, just explaining the presumed mechanics.
(besides, if someone was metagaming on that level, he/she should be aware of Anderson's war asset value)

I wonder how many people know Anderson gives/takes away assets? It took me a long time to figure it out.

Hate to be pedantic but the stories of her bathing in blood are based on accounts recorded years after her death and highly unreliable. Don't get me wrong, she wasn't a nice woman. Evidence entered into her trial suggested she could have had as many as 650 victims, which would make her far and away the most prolific serial killer in history (the evidence could not be proven and the count of her victims is 80 which is still a hell of a lot). But the tale that she bathed in the blood of virgins to retain her youth is most likely untrue.
