The whole thing is confusing. On PC here, and the SM (almost) never plays out exactly like it should according to that diagram.

 

If someon has been able to get 4 to die I would like to know

I believe I had that on my dedicated "everyone dies" playthrough, but it has been really long since then, so I don't remember it all too well.

 

Everyone recruited, none loyal (maybe apart from Kasumi)

3 dead because of missing upgrades -Jack, Kasumi and Thane, I think (Maybe it hit Samara instead of Thane)

Jacob died in the vents

I think I lost Mordin to the seekers -even though it should have been whoever else I had with me (Garrus?)

Grunt lead the second fire team and died.

No escort for the Crew and I took Garrus and Zaeed with me to the reaper.

That should have left Samara or Thane, Legion, Miranda, and Tali at the Barricades -who all died.

 

Edit: According to that chart this should result in 2 deaths - even if I did not actually activate Legion in that playthrough (not entirely sure). But I am certain that everyone died except for Joker.

I will try that on my next ME2 playthrough, though its hard to believe those 4 died holding the line when I had Jack, Kasumi, Mordin and Tali, the 4 weakest and unloyal, holding the line and only 3 died.

Always, I think. But this conversation has made me unsure of everything Mike didn't say he had no upgrades though

Jack will always get lazored if you don't have the armor upgrade.

Yes she does. I had the upgrade and my playthrough was to purposely see if I could have 4 dead while holding the line with the weakest characters, who happen to be Jack, Kausmi, Tali and Mordin. I was only able to have 3 die.

I find this highly useful when planning for suicide mission... either for survival or dying of certain people

I love that chart.

Note that a few PS3 players have posted that that chart is inaccurate. Can't verify that for myself since I play on PC.

I knew Ruthles Shepard was a tad extreme, but I'd never heard this before:

 

 

Batarian propaganda. Think about a Colonist Shepard who went in there. They probably had slaves in their torture chambers. Shepard lost his/her family and friends on Mindoir. Perhaps some were taken by the Batarians. This is what those would have endured. Payback is a b*tch.

this was a big fat glaring thing I discovered. I have finished ME3 for the first time (and sadly have to go back and get 1 and 2) and when I went on the push to the beam I took Kaidan with me. I figured that everyone had died (obviously not Anderson). When the Normandy lands on Utopia or Eden or Heaven Joker exits (Yay! love Joker) and then Kaidan. Bug?

I just noticed that the map on the display behind Petrovsky after Shepard disables the reactor and he says "get the demolition units into position" is a layout of Afterlife and the Market district.

I have never discouraged a romance between EDI and Joker before, so this dialogue was new to me.

I am not sure this was mentioned before, but Javik will ask "why haven't you thrown the 'Legion' machine out the air lock" even when playing with not-Legion.

I just had that dialogue a few minutes ago amd he never said that when I talked with him. I wonder if he says that if Legion didn't survive the suicide mission. On this current playthrough, I gave the Geth to Cerberus.

A) I didn't give Legion to Cerberus, I simply never activated him.

 

B ) This is dialogue you get when activating Javik after the long dialogue about Zhatil, etc.
