I never gotten them either. IIRC, there are very specific circumstances that trigger them.

They've acknowledged there are some bugs in the eyecolors for now. 

It's just a bug. One of us asked Liz Lehtonen if they changed his eyes and she said they hadn't.

 

Oh, alright Since they gave everyone else brown eyes (Leliana, Solas) and then said they were bugs I thought this one was intentional.

 

It was mentioned in twitter by Mike Laidlaw that there is currently a bug related to the character's eye colour,so I expect him to have his hazel eyes again by the time the game is released.  
Also you warden looks beautiful!
